Are you a warm, dynamic individual who strives to give children and young people the support and guidance needed to grow and develop.
We have an exciting opportunity for a Registered Nurse or Allied Healthcare Professional to support the transition of young people with Life Limiting or Life-Threatening Conditions who access hospice care and are transitioning from Children’s to Adult Social Care, Education and/or Health Services.
As part of this role, you will build relationships with adult partners, to support the transition of young adults with progressive life-limiting conditions from Andy’s Children’s Services into appropriate adult palliative care services. You
will also support the development of pathways and resources.
You will lead on Transition between children’s and adult services for the organisation, acting as a resource to colleagues regarding transition issues and offer specialist advice as required. This may include offering training and support to other professionals in supporting young people with complex needs.
As a Registered Nurse or AHP it is essential that you hold a registration with the Nursing and Midwifery Council or Allied Health Professional Qualification on the HCPC register in addition, would be advantageous to possess post registration experience in working with young adults.
This role will require you to travel across all Andys catchment areas so a clean driving license and access to a vehicle is essential. Flexibility is also required to be able to undertake visits at a time convenient to the young person and their family.
As the Director of Clinical Services, I very much looking forward to working with our new Transition Nurse. Nursing is a tough job, having dedicated my life to nursing for over 30 years, but working in palliative care has been one of the best decisions in my nursing career.
