There is an exciting opportunity for a senior band 7 pharmacy technician to lead and manage the provision of a pharmacy technical service to the intensive care units and theatres at the Royal Free Hospital. The successful candidate will be responsible for all technical aspects of medicines management in these areas and for development and delivery of the pharmacy technical service to these areas.

The post holder will consistently work in an accurate, effective, and efficient manner to support the provision of a high-quality service to patients, trust staff and departments. They will be part of a high performing team that is expected to demonstrate the Royal Free NHS Foundation Trust values. The Intensive care unit at the Royal Free Hospital comprises of 50 intensive care beds with over 1500 patients admitted to ICU each year. The Royal Free hospital is a major tertiary referral centre for medical and surgical specialities with the critical care directorate providing services to support all our inpatient specialities including hepatobiliary, surgery, hepatology, complex vascular surgery, infectious diseases and haematology. Royal Free has an active organ transplant programme and the department of intensive care medicine cares for more than 100 liver transplant recipients each year.

The main role of this post holder will be to establish and manage the provision of a pharmacy technical service to the intensive care units, theatres (including day surgery) and resuscitation team at the Royal Free Hospital.

In addition, they will also establish and manage the provision of a clinical technical service to the intensive care units at the Royal Free Hospital and be responsible for all technical aspects of medicines management. This will include helping to complete the medicines reconciliation process for all patients and ensuring safe, appropriate and timely supply of medicines. They will also take responsibility for appropriate supply of stock items to the wards and introduce stock lists for controlled drugs along with a robust and secure method for ordering and supplying them. Regular review of all stock lists to improve stock holding efficiency will also be a key role for this post.

Clinicalresponsibilities
• · In conjunction with the other members of the critical care pharmacy team to provide a full and comprehensive dispensing service to patients attending the intensive care units and theatres. The postholder will ensure maximum efficiency, safety and economic drug use through adherence of Trust policies and demonstrate professional accountability to enhance patient care.
• · These responsibilities include:
• - Delivering an efficient ward-based service to ensure the accurate and timely supply of medicines
• - Accurate medicines reconciliation through GP communication
• - Medical notes interpretation
• - Assessment of patients’ own drugs and confirming accuracy with the patient or relatives where appropriate.
• - Resolving any discrepancies with the prescriber or referring them to the pharmacist
• - Supplying medicines promptly for all ICU patients

· To analyse and interpret prescriptions within intensive care and either resolve problems with the prescriber or refer problems to the pharmacist.

· To ensure the hospital formulary is adhered to and to provide advice and offer solutions when non formulary/unlicensed drugs are prescribed.

· To ensure adherence to hospital antimicrobial policy as well as other clinical protocols for medical patients.

· To promote high-quality cost-effective prescribing.

· To provide comprehensive pharmaceutical advice and information to other health care professionals in highly specialist clinical areas.

· To participate in clinical audit of prescribing, administration and supply of medicines to the intensive care units.

· To contribute to identifying risk issues relating to medicines usage in highly specialist clinical areas and to suggest ways of minimising these risks.

· To maintain proficiency as an accredited checking pharmacy technician (ACPT) and ensure minimum 8 hours a month of checking in line with the requirements of the accreditor.

· To work across the Trust sites or other dispensary areas as per service requirements.

Responsibility for Patients
• To accurately prepare and check prescriptions in a timely manner in response to inpatient requests and discharge prescriptions.
• · To liaise with primary care healthcare professionals to ensure continuity of care for patients on discharge.
• · To support the provision of patient focused activities whereby patients are provided with timely, accurate and understandable information about their medicines during their hospital stay, taking into account the patients’ needs.
• · To remain composed, professional and effective in highly distressing or highly emotional circumstances which arise frequently on ICU.
• · To assess patients on their ability to take medication and provide compliance aids where compliance is a problem. This may include reminder charts, easy-open bottles or multi-compartment compliance aids (MCA’s).
• · To deliver a high quality and comprehensive medicines counselling service to patients on all aspects of their pharmaceutical care including specialist and unlicensed drugs. To overcome communication barriers using interpreters or other forms of communication.

Responsibility for Policy and Service Development
• · To be responsible for the strategic planning and organisation of a technical pharmacy service to the intensive care units, theatres (including day surgery) and the resuscitation team at the Royal Free Hospital. This will include auditing the current service provided (e.g. drug wastage, waiting times, customer satisfaction surveys), producing / implementing strategies to improve / develop the service and will require a high level of initiative. A flexible, innovative and effective approach to unpredictable work patterns is essential and the post holder will need to prioritise the workload based on the needs of the service and stakeholders.
• · To develop and implement systems to ensure effective stock control and ordering on theatres and the intensive care units. This will include implementing pharmacy-led ward top-up services, training staff on how medicines should be put away, checking expiry dates regularly, reviewing stock lists and improving drug wastage and use of drug returns.
• · To develop, implement and maintain SOPs for own area and to contribute to critical care and theatres medicines policies.
• · To manage and co-ordinate responses to drug alerts, shortages and recalls for theatres, ICU and resuscitation and thereby ensuring that patient risk is limited.
• · To assist the senior pharmacists for critical care in the preparation and analysis of monthly drug expenditure reports (for both ICU and theatres).
• · To assist in the development of discharge procedures from ICU to ensure that medicines are transferred with the patient (where appropriate) and not wasted / reordered unnecessarily.
• · To audit current practice relating to medicines management and monitor how medicines are ordered, supplied, used and discarded in theatres and on the intensive care units with a view to implementing measures to reduce wastage, improve stock control and produce cost savings for the Trust.
• · To plan and develop the role of the MMT team in patient-focused care in accordance with local and national strategies.
• · To facilitate and support the development of medicines management initiatives complying with CQC standards for our patients with complex pharmaceutical care needs.
• · To work with managers and colleagues to identify, manage and minimise risks within the Trust risk management frameworks.
• · To investigate medication incidents where appropriate, respond and provide feedback, and implement appropriate actions.
• · To be responsible for regular controlled drug checks and stock audit checks on relevant wards.

Responsibility for Financial and Physical Resources
• · To promote and ensure maximum efficiency, safety and economic medicines use.
• · To assist the team in the preparation and analysis of monthly drug expenditure reports and to resolve any ambiguities as requested.
• · To establish and maintain a robust system for tracking patients who have been prescribed tariff excluded drugs in theatres and ICU so the Trust is appropriately reimbursed.
• · To extract, analyse and interpret data from the pharmacy system using Crystal Reports, including information for KPIs, CQUINs and expenditure reports as required.
• · To develop and implement systems to ensure effective stock control and ordering at ward level. Including ward top-up services, review of stock lists and improvements in drug wastage and use of returns.

• Responsibility for leading and managing
• · To be responsible for the management, supervision, co-ordination and development of MMTs and MMPAs working within the ICU & Theatres team cross-site. This includes recruitment, evaluation of work, identification of training needs, appraisal and professional development.
• · To be independently responsible for the day-to-day management of the critical care pharmacy technical team, including work planning and resource allocation taking into account unpredictable work patterns, staffing changes, interruptions and the need to meet deadlines.
• · To act as an effective role model and to provide pharmaceutical advice and support to members of the multidisciplinary team.
• · To lead and co-ordinate the MMT and dispensing team when required in ensuring discharge medications are prepared in a timely fashion from the pharmacy dispensary.
• · To support an innovative approach to practice, whilst being aware of boundaries and limitations.
• · To identify, undertake and co-ordinate audit projects and publish or present these findings. To assist other members of the pharmacy team in audit projects.
• · To assist pharmacists covering allocated ward during their rotations.
• · In conjunction with the senior ward staff, be responsible for maintaining a high standard of medicines management including regular audit, in order to ensure consistent compliance with national standards.
• · To participate in the appraisal of staff including objective setting, personal and career development of team members and ongoing review of objectives.
• To participate in recruitment and selection of pharmacy staff.
