The post holder will play an active role as a member of the multidisciplinary team. The post holder will delegate clinical tasks to health support workers, trainee nursing associates, student nurses, student Allied Health Professionals and Trust volunteers The post holder will act as clinical supervisor to health care support workers and adhere to Trust policy relating to clinical supervision The post holder will have responsibilities for collaborating and communicating effectively with service users carers and families. This will require the person to manage and engage with sensitive and sometimes, distressing information. The post holder will have responsibilities for communicating both written and verbal information to the team on a daily basis, and the ability to assess, deliver and monitor care and communicate this effectively within clinical case notes of service users under the supervision of a registered nurse. The post holder will participate and contribute to multi-disciplinary reviews, care programme approach meetings and other clinical meetings communicating assessments and delivery of care as part of the overarching care package. The post holder will in line with NMC standards of proficiency be responsible and accountable for their actions and ensure they maintain the necessary requirements to register and revalidate. The ability to manage personal and often distressing information with sensitivity. The ability to assess and manage (via appropriate care delivery programmes) stress/distress/anger/aggression The post holder will have an understanding of the importance of relational security and be aware of the risks which can have an impact when working in forensics. Ability to recognise any deterioration of mental health conditions which will require more intensive support options, along with ability to assess severity and ensure this is referred on to the most appropriate service/professional for follow-up. The post holder will have the skills and knowledge to identify potential safeguarding risks to both vulnerable adults and children. They will have the ability to act on these concerns in accordance with both organisational and local authority safeguarding procedures To carry out physical health interventions including venepuncture, ECGs wound care and medicines administration commensurate with level of training received. To hold a defined caseload of service users which is supervised and monitored by a registered practitioner. To develop individualised, collaborative plans of care to meet the immediate and ongoing needs of these service user and their carers, under the supervision of the registered practitioner. These may include interventions that support registered nurses in the administration of medication. Have demonstrated the proficiency to administer medication through successful completion of the nursing associates training course and registration with the NMC Exercise professional accountability in ensuring the safe administration of medicines to those receiving care Administer medication via oral, topical and inhalation routes. Administer injections using subcutaneous and intramuscular routes and manage injection equipment Administer and monitor medications using enteral equipment Administer enemas and suppositories Manage and monitor effectiveness of symptom relief medication Recognise and respond to adverse or abnormal reactions to medications, and when and how to escalate any concerns Undertake safe storage, transportation and disposal of medicinal products. Day to day management/supervision of health support workers including providing supervision, appraisals, delivering training in own work area, managing workload and attendance etc. Provide peer support to newly qualified nursing associates and buddying to trainees and Band 4 staff.
