Summary

Duties and Responsibilities: The duties and responsibilities to be undertaken by the Macmillan PCN Cancer Care Coordination team may include any or all of the items in the following list. Duties may be varied from time to time under the direction of the PCN Transformation Manager, dependent on current and evolving workload and staffing levels: Develop an understanding of our coding of cancer using clinical templates. Develop and maintain a register of patients with cancer. To work according to confidentiality and data sharing policies of SCORE PCN. Achieve agreed performance targets and participate in regular supervision and personal reviews. Engage in training, identify personal and professional development needs and continuously seek to refresh knowledge in relation to the role. Cooperate in the formulation and development of systems for monitoring and evaluating the work of the project. Liaise with local cancer support groups. Provide a local point of contact for bereaved relatives, signposting towards any appropriate support at any stage following the loss of a relative or close friend. To work across any of the 4 Practice's as and when required. Work within the Clinical Governance framework of the practices in the PCN, incorporating risk management and all other quality initiatives. Where required and appropriate, refer people back to other health professionals within the PCN. Support staff within the PCN with information about local voluntary and community resources and how patients can access them. Treat with confidentiality any information about any organisation, its staff, trustees, or clients that is sensitive, personal or private. Develop and review care plans to include: Being involved in the introduction of and changes to care plans. Ensuring care plans and advanced care plans are up to date and shared with the relevant people. Ensuring that all patients are discussed, and outcomes are recorded and reviewed. Working with staff teams to ensure any planned or recommended treatment occurs in a reliable and timely manner acting as a point of contact. Informing the Lead Clinician of any delays in assessment or delivery of care needs. Identify where plans or recommendations are not achieved and undertake process mapping to identify any bottlenecks.
