Are you interested in developing and promoting health and wellbeing to Ceredigion residents? Are you organised, motivated and enjoy working with people with a range of health needs? Are you passionate about providing inclusive opportunities that can contribute to improving the physical and mental wellbeing of the residents of Ceredigion? If you are then please read on.

We are looking to recruit 2 x Active Lifestyle Mentors to join our Wellbeing Centre Service who will:
- be responsible for delivering and monitoring all Exercise Referral pathways in their local area
- to create and develop opportunities which will support older adults to engage in physical activity
- have responsibility of planning, coordinating and delivering other health interventions programmes and support in a designated area or community. These include Exercise Buddies, Walking for Wellbeing, and Pre-diabetes
- have responsibility to support the Wellbeing and Improvement Objectives in providing robust targeted and open access provisions, including centre based and outreach programmes, throughout the county and in partnership with key statutory and voluntary stakeholders

We are looking to recruit individuals:
- with excellent inter-personal skills and the ability to communicate effectively
- ability to work on own initiative and as part of a team
- a good level of digital/IT skills and excellent communication skills, both written and verbal. E.g staff, public and other professionals
- clear commitment to uphold the principles of equality and diversity
- experience of recording personal and confidential data using a database
