Crynodeb
Developing an effective discharge plan in collaboration with the hospital, GP and community services and that Hospice rapid access funds are applied appropriately. Visiting patients in their place of residence Providing necessary care, support and advice to patients, family members and other healthcare professionals & that patients are reviewed in a timely manner. Contributing towards on-going quality improvement of palliative care services by participating in audit and research activities Working cohesively within a multidisciplinary team to ensure effective communication and optimal patient care Collaborating with Inpatient Unit regarding use of Rapid Access Beds. Ensuring smooth transition of care provision when ending the Hospice Rapid Access service pathway.
