This is a maternity cover community-based post offers an exciting opportunity to work in the Bexley Memory Service.

This will involve working under supervision to provide a broad range of intervention including physical health checks, carrying out ECG’s, review appointments and supporting the Memory Service post diagnostic program.

The service is based at The Bexleyheath centre, Bexleyheath and comprise of well established knowledgeable multi disciplinary team who work closely with Dementia Advisors.

The post holder will be supported by a qualified member of staff, to carry out a varied range of interventions including visiting clients and their families at home and in clinic, supporting them in living well with Dementia and providing education.

The post holder will be expected to carry out weekly ECG clinics both at base and in client's homes, training to be provided.

They will also carry out review appointments with client's who have received a diagnosis, this work will be carried out under the supervision of clinical staff.

The post holder will be expected to support the post diagnostic group programme, assisting with co facilitation.

The post holder will be expected to take part in audits that support service development along with research.

The post holder will be expected to maintain clinical records, attend and participate in regular supervision, attend team meetings, CPD and reflective practice.

To work with the members of the multi-disciplinary team to support service users in the Bexley Memory service to live well and independently in the community.

The postholder will be responsible to the qualified staff within the Bexley Memory Service and will be given supervision and guidance, although the majority of the time will work without direct supervision.

The postholder will carry out weekly ECG clinics both at base and in patient homes. The post holder will carry out review appointments reviewing medication and physical health, this will be supported and care planned via a specialist memory nurse. They will also be expected to support the post diagnostic group programme.
