Summary

Clinical Duties To assist Dietitians, ENNS and DAPs in their role by: Information gathering; To be proficient in collecting (from a variety of sources) and accurately reporting data on: Anthropometry, biochemistry, haematological and clinical information to help support assessment, monitoring and treatment plans of patients Nutritional information, including intake from a rage of sources including food charts, diet histories and verbal recall Medical, social, psychological, personal, cultural and economic factors affecting the patients Interact with patients, family members and carers by via telephone and email contact, undertaking delegated tasks as directed by the supervising Dietitian working within the BDA Professional Code of Conduct Understands, demonstrates, and provides person centred care Ability to plan and prioritise workload within scope of practice on a day-to-day basis under the direction of the supervising Dietitian/ ENNS/ HEFT Co-ordinator To liaise with staff/ patients to request: Anthropometric measurements Food record charts for patients who require Dietetic monitoring Information relating to personal and cultural food preferences of patients Information regarding feed stock levels To monitor nutritional commercial feed, supplement and ancillary usage/ wastage and report relevant information to the supervising Dietitian/ ENNS To send relevant Dietetic information to clients as directed by the supervising Dietitian. Administrative Duties First port of contact and troubleshooting any immediate problems encountered by HEF patients Booking new and follow-up appointments as requested by the clinical team if appropriate, including negotiation of dates with the patient and creating correspondence Assist the HEFT Co-ordinator with specialist dietetic clinic management, to include a knowledge of booking appointments and recording outcomes Distribution of daily correspondence and scanning/ uploading as applicable Supporting HEFT Co-ordinator as required Typing correspondence to patients, GPs, medical staff and any other required addressee following clinical attendance or other patient contacts as directed by the HEFT Co-ordinator Highlighting to the clinical team members any urgent matters including patient queries or correspondence that need response and taking action Liaising with external organisations relevant to HEFT patient care e.g. contracted supplier Provide direct support to the HEFT Managers and Clinical Team Leads as necessary Ordering of stationery and office equipment as required enabling the service to operate efficiently Monitor the delivery of goods procured and distribute as appropriate General Duties To order and monitor the use of nutritional products (feeds and ancillaries) under the direction of the supervising Dietitian/ENNS/HEFT co-ordinator. To complete relevant patient case notes and input into data systems in accordance with Professional, Trust and Departmental guidelines on record keeping To assist in the development of diet sheets/ information leaflets within the department and maintain stocks To support the induction of student Dietitians during their clinical placement and support Dietitians/ ENNS/ DAPs in preparing/ delivering their training programme as appropriate To participate in departmental audit as appropriate and small projects with the support of the supervising Dietitian/ ENNS and DAP With support, have the ability to carry out routine research activities under the guidance of more experienced staff To be involved in Health Promotion initiatives such as promotional events, study days and patient related activities, under the direction of the supervising Dietitian/ ENNS To maintain strict confidentiality of patient information in accordance with Clinical Governance guidelines To work as part of the HEFT, participating in service developments, service initiatives, attending staff meetings and Clinical Governance forums Has an understanding of credible sources of information, and where these can be accessed To understand and comply with the GHNHSFT framework of Policies and Procedures To accurately record statistics on patient activity in compliance with departmental requirements To provide cover for colleagues during periods of leave under the direction of the Team Leader To assist Dietitians with ward enteral feed discharges including checking on the provision of relevant equipment and feed supplies prior to discharge Registering patient details on the relevant online system Additional Contact: Edie StoneHEFT Co-ordinatoredie.stone@nhs.net0300 422 5645
