Crynodeb
To carry out monthly reviews of clinical coded activity for missing specific chronic co-morbidity information within and outside current activity spell documentation. Completion of proformas for sign off by clinicians and ensuring the updating of clinical coding where necessary. Monitor the completion of missing/incomplete documentation in a patients chart ie, history and evaluation note. To review clinical coded data against health record and other data sources ie CT scans looking for missing specific chronic co-morbidity information within and outside current activity spell documentation. Responsible for raising and completing clinical coding data queries where missing specific chronic co-morbidity information is identified, and liaise with clinicians for sign off where necessary. Escalating non-compliance via appropriate channels to ensure timely data submission. To review regular inconsistency report for missing specific chronic co-morbidities previously coded against activity and complete clinical coding data query where necessary. Conduct regular data validation and monitoring report activities to ensure that the Trust achieves a high level of data accuracy, quality, and completeness for both mandatory data submissions and financial reimbursement. Will identify and highlight trends in reported deficiencies and issues to management for escalation. To ensure review, sign off of clinical coding data queries and clinical coding is updated by relevant coding analyst within deadlines. To keep accurate and up to date records of reviewed data and queries raised via Excel spreadsheets. To deal with general telephone enquiries. To regularly report to the Clinical Coding Service Manager and/or Clinical Coding Auditor on work carried out and to meet performance standards set by the Clinical Coding Service Manager. To locate and source any legacy paper records required for audit purposes. Ensure all case note movements are logged on the appropriate electronic tracer system. To ensure patient confidentiality is maintained at all times. To assist in the development of and adhere to departmental policies and procedures.
