We are currently seeking a passionate Physiotherapist to join our Integrated Stroke Early Supported Discharge and Community Neuro Rehabilitation Service. Our integrated service provides stroke rehabilitation and community neuro rehabilitation support to service users with complex needs.
You will be responsible for managing a clinical caseload across the North of Cambridgeshire & shaping the service to support the development of your colleagues. You will provide comprehensive assessments and deliver evidence-based specialist neuro Physiotherapy programs to support patients with physical, cognitive & psychosocial needs.

You will provide comprehensive assessments and deliver evidence-based specialist neuro Physiotherapy programmes. You will work as part of a multidisciplinary team in supporting patients with physical, cognitive, and psychosocial needs to achieve their potential.
The ideal candidate should be flexible and adaptable to support service delivery.

Please note for this role you will be required to travel independently around the county meeting strict time deadlines. 1. To assess, plan and develop specialist Physio programmes for patients with complex neurological problems and a wide variety of complex needs with support from advanced practitioners when required.
2. To work as an autonomous practitioner within the integrated multi-professional Integrated Stroke ESD and Community Neuro Rehabilitation Team
3. To be responsible for own caseload and undertake assessment of patients with complex neurological problems and multiple pathologies using specialist clinical reasoning skills.
4. To identify patient needs, agree goals and provide appropriate therapy interventions relating to their physical, functional and cognitive needs. Providing rehabilitation programmes and adaptive equipment to support self management.
5. To be professionally and legally accountable for all aspects for your own and delegated work.
6. To work within Trust, CSP, andHCPC guidelines and to have a good working knowledge of national and local standards of practice in line with legal and operational requirementsand use own professional judgement to vary from established practices whererequired.
7. To undertake holistic assessment of patients with neurological problems (including those with complex presentations and cognitive problems) making use of specialist clinical reasoning skills.
8. To use evidence-based practice, to ensure clinical care is effective andappropriate.
9. To promote independence and wherever possible to avoid hospital admission and reduce inpatient length of stay to ensure those patients receive theappropriate carein the mostappropriate setting.
