Are you a Registered Mental Health Nurse looking for an exciting challenge in an Integrated Mental Health Team, please apply.

The service mission is to provide a high quality service for people with severe and enduring mental health needs by working in active partnership with service users, carers and with local voluntary and community groups, in addition to statutory services within the Primary Care Network (PCN) Structure in Newham. The 9 Newham PCNs are grouped in 6 Mental Health PCNs, with 6 aligned CIMH Teams, managed by a North and South sector manager.

Supported Transfer of Care (STC)

Supported Transfer of Care is the name for our new approach to supporting service users step down from a Community Team into Primary Care. This pathway between CIMHS/CRT and Primary Care will use previous Extended Primary Care Liaison (EPCL/S) service as a starting point and inspiration, but will grow beyond it.

This is an exciting opportunity for an experienced mental health practitioner to develop practical experience of service design and implementation of a new care pathway. The individual will need to demonstrate sound relevant clinical experience, but also have the appetite to develop their non-clinical skills around , implementation and co-design. The successful candidate will have the support and supervision from a CIMHS/CRT Operational Team and the Directorates Senior Programme Manager.

• Facilitating Step up & Step down from Primary Care (including ARRS MHPs)
• Primary Care consent
• A focus on practice-level working, as well as PCN
• Care which is centred around Meaningful interventions
• Named Worker model
• Maximizing Recovery tools; Personal Health Budgets, Recovery Colleges and Dialog

• Establishing positive relationships and communication with all Practices & PCNs in Newham
