An exciting opportunity has arisen for a motivated and enthusiastic pharmacist with excellent clinical and interpersonal skills to lead a high-quality clinical pharmacy service to Adult Diabetes Services at UCLH.
Adult Diabetes Services are part of the Medical Specialties Division, which incorporates a wide range of different clinical specialties, with both an inpatient and outpatient focus.
The Adult Diabetes Service consists of a multidisciplinary team of consultants, specialist registrars, associate specialists, clinical nurse specialists, dieticians and podiatrists working together closely to support and improve the health of people with type 1 and type 2 diabetes across inpatient and outpatient care settings.
The post holder will work as part of the Medicine and Emergency Services pharmacy team to deliver a comprehensive clinical pharmacy service to Adult Diabetes Services, and where relevant the Medical Specialties Division, and collaborate with primary care and other organisations across the North Central London Integrated Care System, to improve patient care in hospital and community settings.
You will lead the day-to-day clinical pharmacy service to the UCLH Adult Diabetes Services, working closely with the Adult Diabetes team and wider MDT to improve the care of patients with type 1 and type 2 diabetes.
You will attend relevant MDT clinical huddles and ward rounds to promote safe practice by reducing medicines related harm during transitions of care, and drive cost effective and evidence-based practice.
As part of the diabetes MDT, you will use your clinical knowledge, skills and experience to inform therapeutic decision making, and use your independent prescribing skills to optimise diabetes medication regimens.
You will act as an expert resource and a point of contact for the diabetes MDT, other UCLH staff, and patients requiring information about diabetes medicines.
Through service evaluations and quality improvement projects, you will work to develop and enhance our existing systems and processes in relation to safety, efficiency, and productivity using Epic®, the UCLH Electronic Health Records System.
You will develop, implement and maintain clinical pathways relating to complex regimens or the use high-cost drugs by working collaboratively with local primary care organisations across the NCL ICS geography to improve patients’ care in hospital and community settings.
