To deliver high quality, skilled nursing care to patients at home and provide rapid response, rehabilitative and post-acute carein a varietyofcommunitysettingsincludingweekendsandbankholidays.The service operates through agreed shift patterns spanning 8am -10pm7daysaweek.



Toutilizespecialist skillsandknowledgetoholisticallyassessneedsand provideshort term programmed of nursing care for vulnerable adults basedonevidencebasedpractice.



To focus on service priorities of admissions avoidance and timely supported discharge.



Totakeresponsibilityformanagingthecaseloadandtheteam,withthe supportoftheTeamLeader.



Tosupplementlocality based planned care team and otherhealthserviceswhenacuteorconcentratednursing care isrequired.



Long days. 12 and half hours shifts with 1 hour break. Either 8 till 20:30 or 10:00 till 22:30.
• To provide skilled, effective and evidence based nursing care to patients in a variety of community settings.
• Toassessreferralsmadetoservice,prioritiseandrespondaccordingto priority,asappropriate bothintermsoflevelandurgency. To respond within agreed timescales as appropriate.
• To use enhanced skills and knowledge to assess physical and psycho­ socialneedsanddevelopacomprehensiveandconcordantplanofcarewith patients, carersandthewiderMultidisciplinaryTeamasneeded.
• To make risk assessments, establishing the safety or otherwise of patients' ability to stay at home, or return home safely from another care setting,providingandarrangingnecessarynursingandsupportivepackages.

· Provide autonomous clinical care to a range of patients presenting with undifferentiated, undiagnosed and/ or complex health needs and long termconditions.

Both verbal and written communication skills, and experience of team working will be essential to the role. Strong interpersonal skills will also be essential. You must be an NMC registered, UK based nurse to apply for this role, as it is 'zero-hours' it does not attract visa sponsorship. You must also have at least 6 months acute experience.
