Our Supported living services help people to live as independently as possible. You will be responsible delivering support for people to live in a safe, excellent quality and happy home.

The Hours
Individual needs come first, and so we need a lot of flexibility when it comes to your working hours. These will be scheduled Monday-Sunday on a rota basis with the rest of your team and can include sleep-ins; waking nights; weekends; and Bank Holidays. Whilst these are our standard shifts, we can accommodate flexible working requests and part-time hours to meet personal requirements. This can be discussed at interview.
You will:
· Develop a relationship with individuals, their families and circles of support in order to determine what support they may want with their available funding.
· Use the range of person-centred thinking tools and resources to develop an understanding and a ‘picture’ of what support people want/need.
· You will to create a framework for decision making that incorporates the wishes of the individual, their family and circles of support, fom this you will facilitate and develop a support plan with the individual, which identifies agreed outcomes and is in line with their available funding
· Assist and support individuals to arrange a person-centred review of their support plan.
· Explore flexible and creative support opportunities which may result in individuals becoming less reliant on learning disability services.
· You will take the key role of a community connector and match the skills and interests of individuals with opportunities within their local community.
· For those individuals that are interested in employment opportunities, receive the necessary advice, support and guidance in order to ensure successful outcomes.
· Respond and report on any incidents or concerns that arise during their stay, in accordance with health and safety, safeguarding and other relevant incident management processes.
