An exciting opportunity has arisen for a dynamic, motivated community Clinical Sister to work across both primary and secondary care, ensuring effective discharge planning of complex patients who are deemed clinically stable, facilitating and influencing the discharge process/transfer to primary care.

You will need to be able to work in an autonomous way and facilitate effective communication between ward staff , the integrated discharge team and social care team, to promote a culture where transfers of care between primary, community and secondary services are safe, timely and well co-ordinated for patients who no longer require acute hospital care.

Part of the role will involve working within the Community nursing teams proactively supporting and case managing a caseload of complex patients with a range of long term conditions, who are at high risk of unplanned admission to hospital or prone to unnecessarily long lengths of stay.
• You must be able to travel throughout the working day across the borough.

Walsall Manor Hospital houses the full range of district general hospital services. The £170 million development was completed in 2010 and we are continually upgrading. Construction of the new integrated critical care unit was completed in2018,extension of a Neonatal Unit and brand new, state of the art Emergency Department is in the process of being completed.

RESPECT - COMPASSION - TEAMWORK-PROFESSIONALISM
• Use skills and knowledge to assess the physical and psycho – social needs of the patients in their own homes within 24 hours of hospital discharge Including observations, medication checks, and skin checks.
• To work with the hospital team to facilitate safe, timely and appropriate transfers from wards for patients who are known to place based teams
• Anticipate short and long term care needs and develop appropriate care plans for patients in order to prevent unnecessary readmissions to hospital including emergency passports.
• Educate and support inpatient wards in community services including all pathways for in and out of the borough.
