Job Description Job Title: Stop Smoking Advisor Salary: Band 4 £25,000-£27,596 (pro rata for part time hours) depending on experience Speciality : Preventative Services Hours: 15 - 22.5 hours per week (excluding breaks) Employment Type: Permanent Accountable to: Preventative Services Manager Responsible to: COO and PCN Clinical Directors Location: BHNC Head Office, other sites throughout Bexley Borough 1.1 Job Summary This is an exciting opportunity for someone looking to make an impact on the lives of patients living within Bexley and other South London boroughs, to support with multiple projects provided by BHNC within the preventative services team. This team delivers a variety of NHS services including structured diabetes education programmes, the South London Diabetes Book and Learn service (DBL), NHS Health Checks and Stop Smoking service among others. These services support patients to manage their own health and wellbeing, with the aim of preventing hospital admission and to care for patients within the primary care setting. We are looking for a qualified NCSCT trained, Stop Smoking advisor to join the team and work alongside our current advisor, to support with clinics and promotion of the service. This role will work closely with our primary care colleagues, public health and the Stop Smoking team to support the development and implementation of Bexley Stop Smoking Service. A key element of this role is to provide specialist Stop Smoking treatment and support directly to patients at a number of venues in Bexley, as well as on the phone appointments, to reduce smoking prevalence in the borough. 1.2 Key Working Relationships Key relationships include but are not limited to: Internal Relationships Heads of services Project Leads Clinical Service Managers Bexley Practice Managers BHNC administrative team Other clinicians working within the team 1.3 Key Responsibilities Duties To work towards achieving the PHOF relating to smoking cessation. To ensure practice of stop smoking support is informed by the latest evidence base as contained in the PHE Service and Delivery Guidance 2014 and is in line with NICE guidance. To ensure that all health and social care providers are identifying appropriate smokers and referring to stop smoking support. To actively seek out opportunities to maximise referrals to the Service. To develop and strengthen links with primary care and other local partner providers to ensure that effective referral pathways are in place for smokers. Support the Public Health smoking cessation agenda by closely working with the Preventative Services Manager and others involved at all levels of smoking cessation to enable the London Borough of Bexley to reduce smoking prevalence in Bexley. To facilitate high quality specialist stop smoking support on an individual or group basis to smokers living or working in the London Borough of Bexley in order to contribute towards meeting or surpassing smoking cessation targets. This will involve assessing individuals referred to the specialist service to ascertain motivation to quit and providing specialist stop smoking advice, pharmacological advice and carbon monoxide monitoring. Normal working days will be Monday to Friday but a flexible approach to work will be needed as the post holder will be required to provide cover for other Stop Smoking Advisors clinics from time to time. This may mean working later on some days of the week to accommodate service delivery as well as occasional weekend work or evenings. Plan and deliver training programmes to health professionals so that they can provide high quality interventions to smokers in line with NICE guidance and NCSCT. Collect and monitor statistics by inputting data electronically and in a timely manner onto the Quit Manager programme so that progress towards targets can be accurately analysed. Ensure all referrals to the Service are acknowledged and booked onto the programme. Apply specialist knowledge to respond to a range of enquiries regarding the Stop Smoking Service in Bexley e.g. from the general public, community advisors, GPs, hospital staff and local media to disseminate smoking cessation advice and increase the reach of the service. To support primary care staff to share expertise and good practice in order to increase their knowledge, skill and confidence and therefore increase the quality of smoking cessation interventions. To have knowledge of the increased health risks for smokers who have long term chronic health conditions, specifically COPD, pulmonary issues, CHD, cancers etc. and have the ability to motivate and assist them through a supported quit programme. To have knowledge of the risks of smoking during pregnancy and the ability to motivate quit attempts. To plan, organise and schedule presentations to local partners on the impact of smoking to raise greater awareness of the risk of uptake and contribute to a reduction in smoking prevalence. To represent the London Borough of Bexley at London forums when appropriate To ensure that working practices and processes are developed that maximise the use of new technology to ensure efficient and effective delivery of services to residents. Keep under review local and national agendas and initiatives impacting on service requirements and delivery. Identify multi-agency approaches whenever possible. Personal/ Professional Development Assess own performance and take accountability for own actions and development, either directly or under supervision. Contribute to the effectiveness of the team by reflecting on own and team activities and making suggestions on ways to improve and enhance the teams performance. Participate in continuing professional development, education, and training to keep up to date with evidence based knowledge and competence in all aspects of the role, for Continuing Professional Development (CPD) and a Personal Development Plan (PDP). Adhere to organisational policies and procedures, including confidentiality, safeguarding, information governance, health and safety and quality & risk management Participate in any training programme implemented by the management team Effectively manage own time, workload and resources Represent the organisation positively and professionally, behaving as an ambassador for BHNC by displaying the organisations values and levels of expected professionalism at all times. Demonstrate interpersonal skills when dealing with all levels of staff across the BHNC / PCNs using persuasion, tact and reassurance where necessary. Handle confidential patient material in a sensitive and discrete fashion in compliance with Bexley Health Neighbourhood Care policy and procedure. To be able to influence, motivate and involve individuals and teams to reach necessary performance target expectations. Liaise with other departments and members of the Bexley Health Neighbourhood Care and other organisations to ensure the smooth running of the role to assisting the functioning of the PCN team.
