Summary
Key responsibilities This Team Leader role will involve the full assessment of mental health care needs of patients in the community who have been referred to the HTS as they are in crisis/ prevention of admission/ supporting discharge from acute inpatient ward. The role will also gatekeep admissions to Oak Tree ward within working hours The role will facilitate/ oversee HTS urgent response to incoming crisis calls and referrals from Admin/ Triage Lead and will fulfil the role of shift co-ordinator and facilitate incoming phone calls to HTS from patients and their support networks. The team leader will facilitate home visits or respond to need for urgent review in patients own home. They will carry their own caseload of clients and be expected to assess and monitor patients open to the HTS, acting as a resource and taking the lead in the management and delivery of services where appropriate. This post will have a vital leadership role; acting as a role model for others, in communicating and maintaining good working relationships between the different services within OA and the further physical and mental health services. The postholder will take on the full line management responsibilities of a number of staff members within the Home Treatment Service. The postholder will also be the nominated deputy for the Matron of Older Adult Mental Health Service and will provide managerial support to staff within the team. They will also ensure that all services within their area are fully CQC compliant.
