Portsmouth City Council
At Portsmouth City Council we're all about developing our staff, because we want to build a bigger and better city but we need the best employees to do so.
With a little over 207,000 residents and 8,000 businesses throughout this city, this unitary authority's main goal is to ensure the best care and development of all those within the city. From customer service to multi-million-pound projects, our residents are at the heart of everything we do.
The Service
The aim of the Community Connector Service is to reduce feeling of social isolation and loneliness and to increase confidence and ability to access community-based opportunities. This will reduce dependence and demand on health and social care statutory services through opportunities for early intervention. The service supports adults, aged 18+, who are lonely or isolated and may be experiencing poor mental health to identify and access resources and opportunities within their local communities which will improve their connectivity and increase their social support networks. The focus of this service is on early intervention to promote health and wellbeing, help maintain independence and build resilience. Some of our clients have quite complex needs which requires close partnership working across Adult Social Care.
Your New Role
As a Community Connector you will be working in collaboration with the Adults Intermediate Care Team (AICT) based at Queen Alexander Hospital. The AICT is a short-term service completing Care Act assessments and first reviews. You will be liaising and taking referrals from the team along with wider hospital teams, meeting individuals before they are discharged or on discharge from hospital or in the Accident and Emergency Department. You will be supporting clients to identify realistic goals that will enable them to access community opportunities within their local community.
In addition, you will provide support to individuals to achieve their goals e.g. accompanying them to groups/activities/volunteering for a short period of time until they feel confident to do so by themselves. The Community will support individuals to build up their confidence in relation to potential barriers to accessing community opportunities such as using public transport, or confidence with mobility issues or to 'get them through the door' of a new social activity etc.

What you'll need to succeed
You will have a knowledge and experience of safeguarding legislation, excellent IT skills and extensive experience of working face to face in a Social Care or community setting ideally holding a case load. You will have good literacy and numeracy skills and knowledge and understanding of the issues facing people who are experiencing loneliness and isolation and the impact it has on their health and wellbeing.
You will have excellent communication and interpersonal skills and the ability to converse at ease with customers and provide advice in accurate spoken English is essential for this post. You will be flexible to meet the needs of the client including occasional evening and weekend work and a car driver with access to transport or able to easily access public transport to move across Portsmouth City. You will be willing to undertake professional development.
