An extremely exciting opportunity has arisen for a Physiotherapy Lead to support the new and transformational One Coventry Integrated Team (OCIT) service. The trust along with Coventry City Council and Coventry and Warwickshire Partnership Trust have designed and developed 3 Local Integrated Teams (LITs) across the city of Coventry which are supported by mutli-professional teams offering a holistic and integrated approach to urgent community needs for the residents of Coventry.
Local integrated care is one of the Trust’s fundamental purposes, as set out in our refreshed strategy for 2022-2030; ‘More than a hospital’. The Trust has an ambition to deliver improvements in the health and wellbeing of our local populations through collaboration with partners across the health and social care system. The Physiotherapy Lead role is key to the successful delivery of this exciting and transformational service aimed to improve the health and wellbeing of the Coventry population.

The Physiotherapy Lead will work autonomously within the LIT demonstrating effective management skills, providing specialist physiotherapy knowledge and supporting the LITs with the appropriate pathways.
The Physiotherapy Lead will support with admission prevention, hospital discharge and support patients in the community with reablement services.
The successful candidate will have a passion for delivering exceptional community-based therapy, someone compassionate but confident, who can work under pressure and has a desire to play a key role in this new service delivery in Coventry.

• To be an integral part of the local Integrated team (LIT)
• To effectively manage a complex caseload of patients as part of a multidisciplinary team (MDT), providing advanced and evidence based assessment and interventions to patients in both a stable and acute phase of their condition, ensuring the physical, psychosocial, behavioural, perceptual, cognitive, sensory and environmental needs of patients, are addressed.
• Utilising advanced knowledge and clinical reasoning, to plan and provide clinical interventions in consultation with other colleagues in the multidisciplinary team and to work in a range of community settings.
• To adopt a patient centred and sensitive approach to treatment, collaborating with patients (and/or carer(s) where appropriate) in all aspects of the therapy process (assessment, planning, intervention, evaluation, reporting)
• To utilise individual and/or group work skills as appropriate to achieve agreed therapeutic goals
• To have specialist knowledge of a variety of conditions including neurological, orthopaedic, musculoskeletal, care of the elderly, respiratory and falls and advanced knowledge of at least one of these clinical areas
• To work collaboratively with adult social care and third sector providers to ensure effective safe care.
• To support the OCIT Service Leads in the day-to-day management of the service as required.
