Birmingham City Council are recruiting experienced senior social workers into a Hospital Discharge team.
Your new role
Your new role will be undertaking holistic and detailed assessments using a wide range of counselling, interviewing and interpersonal skills. Communicating with patients and others involved in their care, to plan how their needs will be best met. You will be providing appropriate advice, support and advocacy.The successful candidate will support patients with specific health needs, their carers and the multidisciplinary team through the relevant patient pathway.Assess and review patients' changing needs within the specialist area of responsibility by providing continuing care in a proactive and fluid manner to improve the patients' experience.
What you'll need to succeed
The successful candidate will be a very effective and user-focused social worker, someone who is able to use their experience to start this role and be able to be self-sufficient. The candidate needs to use current and key legislation effectively across all of their assessments and reviews. To be successful, you will need experience in adult social work, preferably in Hospital and Mental Health Teams.
A Social Work qualification and Social Work England registration is essential.
