Crynodeb

The post holder will work within their clinical competencies as part of a multi-disciplinary team to provide expertise in clinical medicines optimisation across the PCN. This will include structured medication reviews with direct patient contact and may include contributing to: Management of long-term conditions To support the care homes within the PCN with structured medication reviews To work with colleagues to establish and develop good medicines management systems across the care homes sector To assist the PCN care homes team and wider MDT with care home-related medication queries Liaise with care home staff as necessary to ensure accurate exchange of information and clarification of medication queries Work effectively within the PCN Pharmacy and Care Home Teams across all practices within the PCN To establish and improve professional standards of care in the management of residents within the care homes by developing relationships with key stakeholders Contributing to repeat prescription authorisations and reauthorisation. Action acute prescription requests Addressing both the public health and social care needs of patients Contributing to achievement of QOF and locally commissioned quality improvement schemes. Undertaking clinical audit Developing, promoting and implementing the high quality, evidence based and cost effective use of medicines in primary care in a way that maximises the benefits of medicines to patients and service users and which minimises risks (clinical, legal and financial) This list is not exhaustive and additional duties may be decided by the PCN Leadership Team and the Senior Clinical Pharmacist. The post holder will be supported by existing pharmacists employed within the PCN and a clinical mentor. The post holder will be required to enrol on the General Practice Pharmacist Training Pathway from CPPE (unless an equivalent qualification is already held and exemption is agreed by CPPE) and will also be required to enrol on a prescribing course if a non-medical prescribing qualification if not already held.
