The post holder will working in the High Intensity User Service at the Homerton University Hospital in a fixed-term post from September 2024 for 3 years. The post holder will work with Homerton Psychological Medicine which is the psychiatric liaison serving the Homerton University Hospital (HUH) including the Emergency Department and inpatient wards. They will form part of the High Intensity User Service (HIUS) at the Homerton University Hospital which is coordinated by the HIU Service Lead who maintains strategic oversight of all frequent attenders (FAs), including those to NHS 111 and the London Ambulance Service.
The post holder will carry out the following:
· Led by the HIU Service Lead, participate in the screening of high intensity service users (HIUs)/ frequent attenders (FAs) to develop a clinical cohort for the project.
· Lead in the active case management of HIUs/FAs following screening.
· Maintain an overview of the selected cohort of patients.
· Create personalised and first person care plans for specific HIUs/FAs. Where this is not possible care plans bearing the person and their complexities in mind is created
· Provide expert advice on the management, treatment and diagnosis of patients to acute hospital clinicians – where appropriate for HIUS to seek the expert advice from the relevant clinical specialty such as hematology, renal etc.
· Have an overview of services in the local health and social care economy and signpost or refer on as necessary.
· Provide support to acute hospital clinicians to understand the complexities of frequent attending
· To reduce usage of the Emergency Department, NHS 111 and London Ambulance Service
· To improve the experience of patients who receive services in the above.
