We are looking for a positive and enthusiastic pharmacist to join our friendly pharmacy team at Cavell Centre, Peterborough.

We work as part of Trustwide pharmacy team to deliver a complete medicines service to mental health services and community health services for adults and older people in Cambridgeshire.

We ideally want someone with mental health experience, but we would consider people that don't, but would like to make a move into the mental health specialty. We would support you in that transition.

The post holder would be expected to contribute to a Trust wide 'out-of-hours' service on a rota will all other mental health pharmacists in the team. This requires the successful person to be able to travel to the work base outside of standard working hours.

The person we are looking for is someone that can positively contribute to the clinical medicines management of people with mental health challenges. The successful candidate will work closely with inpatient multidisciplinary teams to support safe and effective use of medicines and support a large number of outpatients through their work with clinics, community team and dispensary services within the Trust.

The successful candidate will contribute to the clinical pharmacy service provided to all inpatient services, and to the dispensary. The role will also involve the handling of medicines information questions, work with clinical trials, policy and procedure development. We expect the person to be able to provide bespoke patient-centred pharmacy care to anyone that requires it from anywhere in the service.

To assist with medicines management service and dispensing service at Cavell Centre Hospital.

To assist with medicines management service at Nort Cambridgeshire Hospital.

To assist with medicines management service at Intermediate Care Centre.

To optimise patient care in defined wards/teams alongside other pharmacy staff by:

a. Participating (in conjunction with pharmacy technicians) in medicines reconciliation and medication interviews with service users.

b. Participating in medicines management schemes on the wards, including patient’s own drugs, self-administration, medicines reconciliation, and medicines supply.

c. Counselling and/or education of service users about their medicines and provide information and advice about medicines to service users, carers and other health professionals e.g., medical and nursing staff.

d. Active involvement in discharge planning for service users to ensure a seamless medicines pathway into primary care.

e. Participating in multidisciplinary team meetings or case conferences to identify medication problems and provide advice.

f. Reviewing and monitoring prescription charts and monitoring the outcomes of treatment.

g. To support prescribing in line with evidence-based practice, following local and national guidelines.

To support the education and training of medicines matters for CPFT staff and service users.
