Crynodeb
With respect to practice rotas, as agreed, the post-holder will make themselves available to undertake a variety of practice duties, including face to face consultations, telephone consultations and home/care home visiting(rare). We would expect duties such like actioning bloods, tasks, checking and signing EPS repeat prescriptions and PA scripts, dealing with queries to be part of the day to day schedule, and dealing with queries. Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation Assessing the health care needs of patients with undifferentiated and undiagnosed problems Developing care plans for patients with long term conditions Providing counselling and health education Prescribing in accordance with the practice prescribing formulary and working to ICB guidelines. Supporting/supervising Physician Associates and GP registar when required ensure smooth running of the practice Manage and Support ongoing palliative care and elderly care case loads.Job description Job responsibilities
