Are you a committed, highly motivated, enthusiastic Nurse or Paramedic?

UCRT is a 7 day service that responds to patients in crisis in their own homes, including nursing and residential care homes, to avoid admission to hospital.

This a national community crisis service model which is delivered across the Mid and South Essex through joint working and collaboration with local partners, such as East of England Ambulance Service (EEAST), Mid and South Essex Hospitals, General Practitioners, Social Care teams, NHS community services and the voluntary sector

The service delivers a holistic assessment to the patient’s home within two hours of referral, identifying health or social care needs and developing assessment and treatment plans to come up with appropriate differential diagnoses.

We are looking for experienced paramedics or nurses with a passion for patient care. You will want to develop your skills and push yourself outside the normal boundaries of practice and have a real drive to make sure your patient gets the best treatment possible. Excellent communication skills and a strong work ethic is essential and in return we offer significant development opportunities.

UCRT works 7 days a week covering shifts between 8:00am until 10:00pm. You will be expected to work weekends and bank holidays although these will be distributed throughout the team.



The post holder will be an experienced Registered Nurse in Adult Nursing or a HCPC registered Paramedic who will work across the UCRT services. We would encourage applications from professionals with high level clinical skills, knowledge and experience in a constantly evolving environment.

For this post, the following are essential
• Current NMC/ HCPC registration
• Reliability and a strong work ethic
• A full UK driving license

As a Band 6 urgent community responder you will:

• Undertake/have the Clinical Assessment & Diagnostic and a willingness to undertake/have the Non Medical Prescribing V300

• Provide good leadership to the team and support the development of the service through leading on projects and audits.
• Be the clinical coordinator on allocated shifts, offering advice and support to colleagues
• Attend patients to perform a rapid assessments and if required for follow up visits.
• Develop your clinical skills into a person centred holistic approach
• An opportunity to develop new skills outside of your normal scope of practice

The post holder will be a Registered Adult Nurse or HCPC Paramedic who will have significant intermediate care experience at Band 5 or 6. Must possess Clinical Assessment Skills and either Non-Medical Prescriber or working towards qualifications. The post holder will demonstrate safe clinical decision making and expert care for the patients registered within the service. The post holder will be professionally and legally accountable for all aspects of care from initial history taking through to treatment. They will be responsible for patients allocated to them and will assess, manage and deliver care to meet the complex needs for those patients requiring intermediate care and primary care services. The post holder will be able to manage and support their peers, working on their own initiative and but also an effective team member along with the requirements of pre-registration students. They will be committed to working collaboratively, actively supporting and liaising with other health, social professionals and other agencies.
