Job Title: Clinical Pharmacist Salary: Band 7-8a (depending on experience) Employer: Bristol Inner City Primary Care Network Hours: 37.5 hours a week Our ideal candidate is a Clinical Pharmacist who has a keen interest in providing long term conditions management with our patients. You will be able to practice independently and be a valuable part of the team. If you would like to do your prescriber training, we will support you to do this. If you are interested in this role as either an independent practitioner to you are willing to do the training for the role we would love to hear from you. The post holder will be working as part of the Bristol Inner City Primary Care Network, a group of 7 surgeries passionate about working with our inner city population: Home (gpweb.org.uk). The purpose of the role is to support the general practice workforce and improve patient care at Montpelier Health Centre. We are keen that this role develops in line with the national Clinical Pharmacist programme: https://www.england.nhs.uk/gp/gpfv/investment/gp-contract/briefing-on-clinical-pharmacists-and-the-network-contract-des/. The post holder will be supported to complete the CPPE Pathway: Primary care pharmacy education pathway : CPPE. Role summary: Working as part of a multi-disciplinary team in a patient facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas Supporting patients and the practice(s) with chronic disease management. Undertaking clinical medication reviews Managing prescription processes and medication queries Providing clinical leadership on medicines optimisation and quality improvement (including QOF) Ensuring the integration of pharmacists across sectors Development support and mentorship will be provided on a practice level, across the PCN and when available on a locality/CCG level. You will be working alongside a team of pharmacists whose roles will be similar to your own and shared across the Primary Care Network(PCN). Practices within our PCN already employ clinical pharmacists and have a track record of supporting new clinical pharmacists develop skills, expertise and become independent prescribers. This post will also include an accredited training pathway for pharmacists to be able to practice and prescribe safely and effectively in a primary care setting (currently the CPPE Clinical Pharmacist training pathways), and in order to deliver the key responsibilities outlined below.
