The post holder will work as an integral part of the established specialist Lung Cancer Nursing Team supporting adult patients with an Lung cancer diagnosis referred to Calderdale & Huddersfield NHS Foundation Trust. The post is for a specialist autonomous nurse who will be responsible for providing specialist nursing care to patients and their family’s diagnosed with a Lung cancer.
We have two part time posts available. 22.5 hours and 30 hours.
The post holder will organise and manage a caseload of patients. Communicating with patients, and their family, assessing their needs and providing relevant support, information, education, and advice as required. The post holder will be providing a service to patients from diagnosis and throughout the course of their disease and for those living with and beyond their cancer diagnosis. The role requires the ability to make appropriate and comprehensive referrals to services such as Specialist Palliative Care, Macmillan Specialist Dietitians and community teams such as District Nurses, Occupational Therapists our Macmillan cancer information centre, among others.
The post-holder will support the delivery of a comprehensive nurse-led service including nurse led clinics. They will provide a holistic approach to care, evidence based that meets the individual needs of Lung cancer patients.
There is a need for the post holder to work closely with acute colleagues such as those caring for our patients on the wards and acute specialist teams such as palliative care and acute oncology. The post holder will form part of a wider multi-disciplinary team (MDT) and act as a specialist resource, for all patients diagnosed with one of the afore mentioned cancers in Calderdale and Huddersfield. They will provide a holistic approach to support as per the Personalised Care Agenda and be able to signpost patients to appropriate local services.

The post holder will also provide an essential link between Calderdale & Huddersfield NHS Foundation Trust and the cancer centres which support our patients to ensure seamless care for patients receiving care/treatment in both centres.
A vital part of the role of Lung CNS is helping to prevent hospital admissions for patients. This will involve co-ordinating further investigations and treatment.
The post holder will also be responsible for contributing to education of colleagues and students where needed. This maybe talking on study days or supporting staff unfamiliar with caring for Lung cancer patients on the wards or in the community.

Main Tasks
• Provide the core components of the clinical nurse specialist, which includes: clinical practice, education, research and audit, taking in to account the national cancer agenda and its impact on service delivery.
• To work within the NMC code of professional practice, and within policies, procedures, and guidelines of Calderdale & Huddersfield NHS Foundation Trust.
• To ensure that high standards of nursing care are given and maintained and act when standards are not being maintained.
• Act a clinical role model, ensuring clinical expertise and practice is enhanced.
• The post-holder will support the development and evaluation of the Lung cancer service to improve and maintain standards of patient care and promote a seamless service within a multi-disciplinary framework
• Work towards being a clinical expert in Lung cancer with good communication skills. The role requires the ability to work autonomously and independently but within an integrated collaborative team.
