1. SCOPE AND RANGE Support an allocated group of individuals with complex health and social needs. Develop a therapeutic rapport with patients within appropriate and transparent boundaries. Work in a way that promotes dignity, respect and independence. Involve patients in the discharge planning process and empower them to make decisions. Proactively identify barriers to discharge and liaise with relevant parties to prevent delays in discharge. Monitor the progress of patients and be mindful of bed availability. Monitor delayed and complex discharges to prevent unnecessary or protracted delay. Be aware of the expected dates of discharge and monitor progress in conjunction with the ward round process. Be aware of the patient's imminent discharge plans and assist in supporting the MDT to expedite these plans. Work closely with a number of agencies and disciplines and proactively promote relationships in order to meet the needs of the patient. Assist the MDT to ensure that discharge plans incorporate social factors. Contribute to meetings and handover relevant information to the MDT. Liaise with Social Workers. Liaise with the Occupational Therapy Department regarding functional assessments and discharge plans. Liaise with Advocacy regarding discharge plans. Work closely with the Discharge Team Leader, the Discharge Team and the wider MDT to ensure agreed plans are actioned. Undertake specific responsibilities designated by the Discharge Team Leader. Complete referrals and liaise with the relevant professionals involved. Complete Discharge Planners. Contribute to MDT Needs Assessments. Complete Adult Social Care referrals. Complete Duty to Refer applications. Complete referrals to BCHA - Hospital Discharge Team. Liaise with Housing Support Workers assigned by the BCHA - Hospital Discharge Team. Complete referrals for supported living and residential placements. Assist with benefit applications, paying bills and budgeting. Refer to appropriate agencies regarding debt. Support patients to maintain their homes. This may include cleaning, removing refuse and positioning furniture. Collate data regarding discharge related tasks and maintain a database. Maintain confidentiality in respect of the person. Know how to report accidents and untoward incidents and to do so promptly. Have knowledge of organisational policies, procedures and practices and carry out duties in accordance with them. Be aware of legal issues and have an understanding of the Mental Health Act. Be willing to undertake mandatory and other training as directed by the organisation. Be willing to participate actively in appropriate in-service training which the Discharge Team Leader believes to be of benefit to the individual. Assume co-responsibility for maintaining skills and knowledge levels. Participate in personal development using the appraisal system. Receive line management support and supervision from the Discharge Team Leader. Effectively record information on the patient records. PHYSICAL DEMANDS OF THE JOB Frequent sitting & standing. Inputting at a keyboard, frequent light effort to lift, sort & file documents. Will use VDU for high proportion of time. Required to regularly drive the pool car. Occasionally for long periods depending on the destination. Exposure to unpleasant conditions is rare and or occasional. May be exposed to verbal aggression from patients. May also be exposed to poor living conditions when completing home visits. Occasional support with cleaning, removing refuse and positioning furniture when supporting patients to maintain their homes. 8.
