Ensure the delivery of high quality care through the Dementia Service and demonstrate high personal standards in clinical practice. To ensure care is provided in line with the National Dementia Strategy and associated NICE Guidance, and RCOT Care Cluster guidance including other relevant local and national documentation. Responsible for prioritising workload, allocation of work to oneself and other staff, and to cover all levels of priority of work within the service. Undertake screening assessment of referrals, in line with the Service Specification, to identify if referrals are appropriate and to make clinical judgements backed up by clinical reasoning and recommendations regarding appropriate actions/sign posting in consultation with multi-disciplinary staff. See service users in a variety of locations as appropriate, to carry out specialist assessments (within service specification) to identify mental health needs and if indicated develop an agreed action plan of care. Ensure where appropriate robust risk assessments and risk management plans are undertaken and that positive risk taking is practiced where appropriate to improve quality of life and maximise occupational performance/independence. Deliver a range of non-pharmacological interventions following specific occupational therapy assessments to enable understanding of any expression of an unmet need and to maximise an individuals abilities. Where appropriate, to care manage/care co-ordinate. Take responsibility for the management of a complex caseload, which includes, specialist assessment (within service specification), care planning in conjunction with service user and carer, evaluation of individual treatment plans (Care Programme Approach and Single Assessment Process). Attend team meetings as appropriate to facilitate a safe and timely discharge. Knowledge of national, regional and local drivers underpinning service delivery including the provisions of the Mental Health Act 1983 (revised 2007) regulations including Mental Capacity Act (2005), RCOT Mental Health Strategy and RCOT Care Cluster Guidelines.
