We have an exciting opportunity to be part of the specialist rehabilitation offered at St Pancras Rehabilitation Unit in Kings Cross.
The inpatient neuro rehabilitation based at St Pancras Hospital has been expanded to include a further 6 beds on Rochester East Ward. The remit of the team is to provide neurorehabilitation and occupational therapy intervention for adults with neurological conditions, primarily stroke and brain injury. A dedicated team consisting of occupational therapist, physiotherapy, speech therapy and psychology are being recruited to meet the intensity of rehabilitation required for these patients. The therapy team at St Pancras Rehabilitation Unit promote recovery, independence and community integration, and enable self-management where appropriate. Work with clients can vary from short episodes of assessment/ treatment to several weeks of intense specialist rehabilitation or complex discharge planning, but always follows a goal orientated approach. At times the post holder may be required to hold a very small general rehabilitation caseload e.g. amputee rehabilitation/ patients with a history of falling. The primary caseload will include Level 2 and Level 3 patients requiring neurological rehabilitation. The team is committed to a clinical governance programme and team members are encouraged to use an evidence-based practice model and to participate in research, quality improvement and audit programmes.

• To manage a complex neurological caseload using evidence based clinical reasoning, critical thinking, reflection, and analysis to support assessments, goal setting, personalised rehabilitation and disability management.
• To work flexibly with all members of the team to deliver an integrated rehabilitation service to clients in accordance with the philosophy and operational policies of the team. At times this will include working with a small number of patients with general rehabilitation needs (e.g. patients with frailty, falls, amputee) in addition to a primary caseload of level 2 and level 3 patients requiring neurological rehabilitation.
• To support maintain and further develop a high quality, efficient and effective Occupational Therapy Service within a multidisciplinary context and in accordance with the philosophy and operational policies for the Organisation.
• To be responsible for the supervision and monitoring of junior staff, including Band 5 OT’s, Rehabilitation Assistants and OT students. Cross-disciple working to assist with supervision and monitoring of staff from other professions may be required.
• To actively engage in a wide range of Continuing Professional Development activities.
The In-Patient Therapy team is based in the St Pancras Rehabilitation Unit located in South Wing at the St Pancras Hospital sitewhich offers a strategic location near the conveniences and transportation hubs of Kings Cross and Camden.
The remit of the teams is to provide neurorehabilitation and occupational therapy intervention for adults with neurological conditions. Clients are over 18 and will live within NCL. Primarily the unit serves patients from the London boroughs of Camden, Islington and Haringey. The existing 17 bedded ward on Oakwood ward has been expanded to occupy 6 additional bed on Rochester East Ward to offer specialist neurological rehabilitation. The post holder will primarily work within the team on Rochester East ward with the B7 physiotherapist, and rehabilitation assistants. There may be occasion to flex to support the general rehabilitation team of Rochester East, which fills the further 12 beds. The team aims to promote independence and community integration, and enable self-management where appropriate. Work with clients can vary from short episodes of assessment/ treatment to longer-term support or rehabilitation, but always follows a goal orientated approach. The team is committed to a clinical governance programme and team members are encouraged to use an evidence based practice model and to participate in research, quality improvement and audit programmes.

Overview of Clinical Responsibilities
1. To be professionally and legally accountable for all work undertaken. To work within the standards and guidelines of the team, the Trust, and professional organisations..
2. To ensure that the client and/or their carer has an understanding of treatment protocols and gives informed consent to examination and treatment.
3. To successfully manage, modify and prioritise a caseload of clients with complex needs, with supervision and guidance as necessary from senior staff, taking a key worker role as indicated.
4. To undertake a range of neurological and/or complex multipathology occupational therapy assessments of clients with complex needs through subjective questioning, objective examination and the use of other resources.
5. To prioritise factors involved in the clients presenting condition through clinical reasoning based on knowledge of current professional opinion, clients prognosis and other physical, social, emotional and mental health issues.
6. To agree personalised goals for treatment with clients and/or their carer and ensure that they are supported through their rehabilitation e.g. key worker, carer assessment and specialist sign posting routinely occurring.
7. To maintain a high standard of clinical practice which is evidence-based, and uses standardised assessment and outcome measures, where appropriate.
8. To deliver evidence based treatment programmes within a team approach in a variety of settings and evaluate and review occupational therapy treatment/interventions within a multi-disciplinary framework and modify as necessary.
9. To deliver routine and specialist Occupational Therapy interventions with clients assessing their occupational needs and addressing quality of life, and physical, cognitive, affective, social, and environmental issues utilising appropriate individual or group treatment modalities.
10. To use a range of interventions within rehabilitation and complex case management e.g. when managing tone, motor recovery, increasing participation in ADLs.
11. To actively identify the need for joint working opportunities
12. To delegate and monitor appropriately work to rehabilitation assistants
13. To work autonomously at times in isolated community situations which vary in relation to local socio-economic and multi-cultural needs.
14. To use knowledge of other multi-disciplinary roles and external agencies to ensure effective referral management and achieve best client care including seamless transfer of care.
15. To have the ability, knowledge and skills required for moving and handling people in hospital and community environments and supporting the formulation of a routine risk assessments/ handovers.
16. To be physically able to install specialist occupational therapy equipment in client homes applying safe manual handling at all times, especially when doing so in restricted spaces
17. To advise and teach the relevant carers/health workers to participate in on-going rehabilitation care programmes for clients, as appropriate and ensure that self-management activities are embedded routinely into rehabilitation and transfer of care plans .
18. To participate in routine multidisciplinary assessments of need across health and social care prior to transfer of care ensuring comprehensive risk assessment and specialist rehabilitation prescriptions are used to support with appropriate and timely discharge preparation and referral onwards as needed
19. To actively be involved in the development of new specialist clinical services within the Team.
20. To be able to manage clinical risk in own caseload and support other staff to comply e.g. manual handling risk assessment and local policy regarding lone working. To know when to escalate risk concerns to supervisor and senior management
21. To assist in the development of local best practice clinical uni-disciplinary and multi-disciplinary guidelines
22. To ensure that client and service records are recorded and maintained as per local guidelines and that IT databases are used
23. To independently or jointly with other professions, write reports outlining recommendation for discharge from the team. These are communicated to appropriate external agencies i.e. referrer, GPs, Social Services and Community Therapists
24. To contribute to the efficient, day-to-day, running, of the team.
