Crynodeb
The Community Matron service is a team of highly skilled advanced nurse practitioners working within Care Group 1 of the Trust (Healthy Lives). The matron deliver reactive & proactive interventions to people residing in care homes across the Hartlepool & Stockton CCG area. The key focus of this team is to ensure that people are cared for in their own home environments & that hospital admission is prevented where possible.
