Crynodeb

The post holder will be responsible for delivery of our new Healthy Families Lifestyle service. They will have a direct impact on improving the health and wellbeing of residents by conducting family behaviour change programmes. The bespoke family programmes will include topics such as, parenting programme, physical activity, health and wellbeing, food and nutrition and signposting. The post holder will use their expertise and knowledge to ensure the correct advice and guidance is provided. Build and maintain supportive and collaborative relationships with local families participating in programmes including keeping in touch between sessions. Share knowledge, provide information and guidance regarding lifestyle behaviours such as healthy eating, physical activity, alcohol consumption, smoking, mental wellbeing, and other areas identified from wider determinant factors. The job will require an excellent level of customer care and motivational skills in order to engage and encourage families to make a positive behavioural change. The job holder will need to have the ability to multitask, support and deal with enquiries in a professional manner and have the ability to holistically work with other services in order to provide a customer orientated service. Maintain good relationships with school providers, school nurserys, school nurses and community groups to ensure families and childrens needs are met. To record, monitor and provide outcomes to support the effectiveness of the programme and making sure they meet the community needs by reflecting on practice, analysing the programme and responding to the need of the community.
