We are pleased to be able to offer an exciting opportunity to work become Lead HIPEC Theatre practitioner within the Peritoneal Tumour Service at The Christie. You will be a registered nurse with current NMC registration, or a qualified Operating Department Practitioner with HCPC registration. You will have proven leadership qualities and skills and relevant experience in the management and delivery of HIPEC within the surgical department.
We currently have seven operating theatres including two Da Vinci Robotic theatres and an Endoalpha Olympus Laparoscopic theatre. The successful candidate will join the HIPEC team in providing excellent quality of care to our patients who undergo cytoreductive surgery with HIPEC, for Pseudomyxoma Peritonei, Colorectal Peritoneal Metastasis, Appendix Ca, Goblet Cell Carcinoma and Ovarian Cancer.
You will be required to work 37.5 hours full time, 4 days flexibly over 6 days (Monday to Saturday) with a commitment to out of hours on call. Flexibility to the service needs is essential within this role.

- Deputise to the theatre manager, daily operational running of operating theatres to a high standard, ensuring excellent nursing standards of care are provided across the nursing and AHP workforce.
- Line management duties for varying personnel across the theatre complex, inclusive of HIPEC Practitioners, Surgical First Assistants, Theatre Practitioners, Healthcare support workers, Admin/ Clerical and Decontamination personnel.
- You will need to have good communication skills, be highly motivated, proactive and be able to work using your own initiative. Both high levels of technical and non-technical skills will be required for this role.
- Professionalism in communicating across the wider multidisciplinary team. Presenting and feedback on behalf of the theatres/ HIPEC team in both internal and external organisation meetings and events.



The Christie is one of Europe’s leading cancer centres, treating over 60,000 patients a year. We are based in Manchester and serve a population of 3.2 million across Greater Manchester & Cheshire, but as a national specialist around 15% patients are referred to us from other parts of the country.

We provide radiotherapy through one of the largest radiotherapy departments in the world; chemotherapy on site and through 14 other hospitals; highly specialist surgery for complex and rare cancer; and a wide range of support and diagnostic services. We run one of the largest early clinical trial units in Europe with over 300 trials every year. Cancer research in Manchester, most of which is undertaken on the Christie site, has been officially ranked the best in the UK.

The PTS hosts a dedicated multidisciplinary team of nine consultant colorectal surgeons, consultant oncologists, clinical nurse specialists, HIPEC practitioners and many more. You will also work in conjunction with the dedicated Gynaeoncology multidisciplinary team supporting with the growth of their service.
The successful applicant must have proven ability to lead and will deputise within the theatre department to the theatre manager. The role is combined clinical and managerial work. The postholder is required to maintain clinical skills as a UKONS SACT passport holder.

DUTIES AND RESPONSIBILITIES

- Daily operational organisation of the HIPEC service within theatres including Preparing patient PMH, chemotherapy history, height/weight and bloods in an organized and timely manner for prescription. Liaise with oncologists to ensure all prescriptions are prescribed in a timely manner. Ordering of chemotherapy to surgical theatres. Liaising with pharmacy; outside agencies regarding any delivery issues of chemotherapy. Ensuring the patient meets practitioner approval protocol and escalate to oncologist if they do not.
- Clinical delivery of HIPEC -Set up/ maintenance and troubleshooting of the RAND perfusion machine. Administration of chemotherapy regimen as per protocol. Maintain ANTT, IV/ CVC skills. Monitor patient temperatures, abdominal pressures, urine output. Adhere to cytotoxic and COSHH guidance in the disposal of equipment.
- Develop, implement, and review departmental policies, standard operating procedures, risk assessments and Local Safety Standards for Invasive Procedures.
- Maintain assets and be responsible for medical devices including procurement.
- Participate in the training and development of others, providing up to date competencies in line with best guidance for various groups of personnel.
- Carry out, report and act upon clinical audit results.
- Develop, implement and review Value Improvement projects.

- Organisational skills are essential in the timely delivery of theatre/ HIPEC projects.



DUTIES AND RESPONSIBILITIES

• Act as a lead theatre practitioner providing expert specialist advice based on highly advanced knowledge and skill to surgeons and junior colleagues.
• Analyse complex information to maintain the patient’s clinical condition within normal physiological parameters in line with set guidelines and protocols.
• Formulation of appropriate response/care pathways particularly for atypical procedures, or where expert opinions may differ.
• Review patient history/notes to produce a pre procedure assessment providing accurate information for the prescribing oncologist to base his chemotherapy prescription upon.
• Liaise with referring treatment centre to establish patients chemotherapy history
• Liaise with Surgical & Oncological specialties with respect to appropriate chemotherapy drug regimens. Liaise with Pharmacy and outside agencies to ensure timely delivery of chemotherapy drugs for HIPEC procedures.
• Develop, implement monitor and review departmental protocol for delivery of chemotherapy drugs for HIPEC procedures and further subsequent combined surgical chemotherapy procedures.
• Operate and manipulate fine controls/tools e.g. calibration/control of HIPEC perfusion device
• Operate relevant monitoring and safety devices.
• Select appropriate equipment & disposable items for each patient/ procedure.
• Ensure the disassembly and disposal of all used consumable equipment in accordance with Trust and departmental guidelines and cytotoxic policy.
• Strictly observe and perform recognised surgical standards of aseptic technique.
• Maintain, clean and calibrate equipment.
• Anticipate potential delivery complications and troubleshoot in order to prevent.
• To lead as a senior member of the Anaesthetic/Recovery/Scrub/Circulator Theatre team with extensive knowledge in the care of the peri-operative patient.
• Provide professional and managerial leadership to the departmental team. Including acting as role model for junior staff.
• Communicate complex information across barriers of understanding showing empathy and reassurance to staff, relatives and patients.
• Carry out all duties in accordance with their respective codes of conduct reflecting an attitude and appearance that befits the profession.
• Organise the appropriate staffing of the theatre department ensuring staffing levels are adequate for the workload and be aware of changing situations; including ensuring that annual leave is commensurate with service requirements.
• Liaise, communicate and negotiate with colleagues to ensure co-operation in achieving departmental objectives, promoting good working relationships and establishing team spirit.
• Act as or support the Theatre Co-ordinator in the day-to-day operational floor management of the operating sessions to ensure through put of patients requiring surgery.
• Plan and facilitate regular staff meetings to maintain effective communication with staff though out the department.
• Attend Senior Nurse meetings in the absence of the Theatre Manager.
• Be proactive in managing incidents/situations and report them appropriately in accordance with trust guidelines.
• Participate in Disciplinary or Grievance procedures according to Trust policy when necessary.
• Deal with complaints or suggestions by staff or patients as appropriate.
• Ensure that equipment service contracts are up to date and that the equipment receives regular servicing/maintenance.
• Be able to process theatre related patient records via the Theatre Computer System and Theatre Care Document.
• Act as a senior resource for clinical advice and troubleshooter for clinical and technical problems.
• Keep up to date with all nursing issues with particular reference to those, which have direct influence on the work of the Theatre Complex.
• Be aware of relevant nursing research and promote and participate in research initiatives.
• Pursue own educational needs in respect of current practices and future developments.
• Monitor, train and develop staff in order for them to perform efficiently and effectively.
• Identify department requirements for training and educational course places.
• Liaise with practice educators within the directorate and Greater Manchester/Colleges of Nursing to co-ordinate staff development and develop directorate education plan.



CLINICAL GOVERNANCE, AUDIT, RESEARCH AND DEVELOPMENT

• Work within trust guidelines, policies, and standard operating procedures.
• Develop, Implement and review departmental policies, standard operating procedures, risk assessments and Local Safety Standards for Invasive Procedures.
• Instigate and participate in regular R&D activity in relation to the specialist PTS/HIPEC perfusion service. Including management of monthly consumable ordering.
• Instigate and participate in complex audits relating to the theatre department.
• Utilise incident reporting system and participate in investigation of incidents.
• Maintain a competence to practice by keeping the required log book of clinical activity.
• Responsible for the inputting and storage of HIPEC data on individual case basis.
• Produce reports relating to HIPEC delivery as requested
• Undertake equipment testing and evaluation, e.g. specifically looking at any reduction in complications and improved clinical outcomes e.g. alternative HIPEC delivery
• Participate collaboratively within the Directorate in the development of the service in line with evidence-based practice
• Inform and influence clinical governance issues
• Network with colleagues and share achievements / learning
• Gather, analyse and report a wide range of audit and research data and information
• Ensure that departmental and trust policies and procedures are observed for the administration of drugs to prevent drug errors and misuse.
• Ensure that other departmental and trust policies and procedures are adhered to by all staff.


FINANCE / PHYSICAL RESOURCES
• The post holder will be an authorised signatory & responsible for the ordering of disposable & non-disposable items.
• Monitor and contribute to the assessment of the Pseudomyxoma Service budget for specialist hyperthermic chemotherapy procedures for the ATS Directorate
• Be responsible for the safe use of equipment by other members of the team, i.e. trainees / newly qualified staff / medical and nursing staff
• Manage stock levels in all clinical areas
• Lead and conduct trials on new products and equipment and where appropriate after evaluation, initiate the change to new merchandise.
• Be responsible for the security of highly complex, expensive equipment and sterile stock specifically within the Surgical Theatre Department.
• Comply with best practice in the safe use of equipment by self and other members of the team, i.e. trainees / medical and nursing staff
• Assist in the development and delivery of value improvement programmes.
