With the choice in working either from home or our offices, you’ll have the flexibility in working location and hours. This post is on a permanent basis and based in our East Locality. This Social work team supporting adults with physical health conditions or older people with dementia, including supporting discharges of older people from mental health wards. Provides longer term support to people who are in receipt of support from adult social care and either live in the east of the city or whose GP is part of the Primary Care Network in the east. Also supports people living in residential or nursing homes.



What you'll be doing




No two days are the same, you’ll be working with people who come into contact with us to have conversations to discuss what matters to them and others around them. You’ll have a can do attitude, a good awareness of the local information and advice that is available and use this to support people and their carers to solutions, drawing on what they already have around them that is working, or could work. You’ll work in partnership with others across social care, Health, Probation, and Housing teams.



What you need




Due to the nature of this role you’ll need to be registered with Social Work England, hold a valid driving UK licence. Appointment will be subject to an enhanced DBS check.
