Crynodeb

The unit is a Level 3 intensive care unit and is one of the largest in London caring for both medical and surgical neonates. There are 12 intensive care, 9 high dependency and 15 special care cots. Training will be given in all aspects of neonatal care and there is opportunity to attend several combined clinical meetings with other departments. The post holder joins a total of 9 middle grades on a EWTD compliant full shift rota.

Proposed start: March 2025

Main duties of the job

The Neonatal Trainees work a full shift system. Each rotates through the 9-week pattern on an equal basis and is EWTD compliant. This ensures that full holidays and study leave may be taken; the timing of these must conform to that imposed by the rota and study leave remains discretionary. Each doctor is expected to undertake an audit project, and to attend local governance/evidence-based medicine meetings. Furthermore, the senior clinical fellow neonatal paediatrics will be encouraged to develop additional skills based on their interests. This could include teaching, simulation, or research.

The Neonatal Unit at St George’s Hospital is a neonatal intensive care unit (Level 3) with neonatal surgery and is one of three lead centres for South London. South West London has approximately 20 000 deliveries per year of which more than 5000 deliver at St George’s. St George’s currently has 12 intensive care cots, 12 high dependency and 16 special care cots. We have over 5000 admissions per year and > 12 000 cot days. Over a quarter of all admissions have a surgical condition. 8% of babies are less than 27 weeks gestation. We care for all conditions requiring intensive care support except complex cardiac and ECMO cases. Available facilities include conventional and high frequency ventilation, inhaled nitric oxide, echocardiography, therapeutic hypothermia, CFM, EEG, complex imaging, and neuroimaging. Other specialist paediatric services include clinical genetics, neurology, radiology, ophthalmology, infectious diseases, endocrinology, haematology, respiratory medicine, gastroenterology, ENT, orthopaedics, and plastic surgery.

The rota is a full shift system which rotates over 9 weeks. Experience will be gained in all aspects of neonatal care, which includes the routine care of normal neonates, resuscitation, the care of low-birth-weight infants and the intensive care of sick newborn infants including techniques such as high frequency oscillation ventilation and the use of nitric oxide. The unit is the regional centre for neonatal surgery and many babies are transferred to the unit from other hospitals.

The Unit is staffed by 10 Consultant Neonatologists working solely on the Neonatal Unit at any one time. There are 7 Advanced Neonatal Nurse Practitioner who work closely with the Trainees. There are formal teaching sessions, and 2 Consultant teaching rounds each week, in intensive, high dependency and special care nurseries, as well as multi-professional clinical meetings each day. In addition, doctors are encouraged to attend general paediatric teaching sessions and joint meetings with the departments of Obstetrics, Paediatric Radiology, Fetal medicine, and Paediatric Surgery.
