We are looking for an enthusiastic Physiotherapist who has wide ranging clinical experience, good communication and interpersonal skills and a passion for delivering high quality patient-centered care to join our Front door and Frailty Therapy team.
As a senior band 6 physiotherapist you will provide and develop a high standard of physiotherapy services in the Frailty and Front door specialty. where you can build upon your clinical and managerial skills with support from the team leads and your integrated therapy lead.
You will be required to work closely and in an integrated manner with other members of the multidisciplinary team (MDT) to promote independence and to facilitate safe, timely and appropriate discharge
You will contribute to the overall service development of the team and specialty, working with senior team members, professional leads, Head of Therapies and manage, supervise the development of staff members within the team.

To undertake a comprehensive assessment of patients including those with diverse or complex presentations/multi pathologies; using acquired clinical reasoning skills and manual assessment techniques to provide an accurate diagnosis of their condition.
Formulate and deliver an individual physiotherapy treatment programme based on a sound knowledge of evidence based practice and treatment options using clinical assessment, reasoning skills and knowledge of treatment skills.
To take delegated responsibility from the band 7 physiotherapists for managing patients with complex conditions and be responsible for providing specialist physiotherapy assessment and treatment plans.
Assess patient understanding of treatment proposals, gain valid informed consent and have the capacity to work within a legal framework with patients who lack capacity to consent to treatment.
Evaluate patient progress, reassess and alter treatment programmes if required.
To manage clinical risk within own patient case load.
To deal sensitively with patients who have high levels of anxiety and aggression caused by pain, dementia or limited mobility.
To be responsible for maintaining accurate and comprehensive patient treatment records in line with Chartered Society of Physiotherapy (CSP) standards of practice.
To be respiratory competent and actively contribute to the physiotherapy on-call service

To provide and develop a high standard of physiotherapy service in the Frailty and Front door specialty.
To manage a caseload of patients independently, balancing clinical and non-clinical duties in a flexible and responsive manner.
To provide standardised and practice-based physiotherapy assessments, interventions, and give advice using evidence-based, patient centered principles.
To work closely with other members of the multidisciplinary team (MDT) to promote independence and to facilitate safe, timely and appropriate discharge
To contribute to clinical audit and service development, working with senior team members, professional leads, and Head of Therapies
To manage, supervise and develop staff members within the team and wider therapies services
To be a role model for other physiotherapists, demonstrating the Trust values of Respect Compassion and Excellent
To actively be part of the therapy weekend and on-call respiratory services
