A position has become vacant in the Complex Discharge team at Barnet hospital for a Senior Complex Discharge Navigator. This is an opportunity to work with a dynamic specialist team at a senior level,
We are looking for an experienced, self-motivated, and passionate person to join our team . The post holder’s main responsibilities would be to provide oversight, management and support of complex discharges, working with the local Transfer of care hub and wider integrated services as part of the complex discharge team.
The post holder will also hold their own case load of the most complex cases, such cases would include patients with no recourse to public funds, homelessness, placements for patients with complex needs and patients who require the application of the Patient Choice Policy.
As the Senior complex Discharge navigator, the post holder will deputise for the Matron for Discharge and share managerial responsibility of a team of Discharge Coordinators and Patient flow coordinators. They will also be expected to provide ongoing live information pertaining to discharges.
Holding a case load of the most complex patients
Manage and support complex discharges to ensure that patients are discharged to the most appropriate setting in a timely manner
Deputise for the Matron for Discharge and ensure that all monitoring, reporting and escalating is provided both internally and externally to ensure all parties are aware of the position of the hospital. They will also be expected to provide live information pertaining to Non Criteria to reside patients and current delays.
To provide specialist discharge advice to multi professional teams, patients, and their advocates.
To escalate barriers to discharge both internally and externally.

The Senior Complex Discharge Navigator will work closely with the Matron for Discharge, other members of the Complex Discharge Team, Patient flow coordinators and all members of the multi-disciplinary team to ensure the timely and safe discharge of patients. They will also represent the Royal Free London as a member of the local transfer of care hub and the wider North Central London Integrated discharge hub.
The post holder will be expected to deputise for the Matron for Discharge and ensure that all monitoring, reporting and escalating is provided both internally and externally to ensure all parties are aware of the position of the hospital. They will also be expected to provide live information pertaining to Non Criteria to reside patients and current delays.
The post holder will have general oversight of patients that do not meet the criteria to reside in hospital and will provide support in all meetings held to reduce these numbers both at patient and local level and within the wider NCL.
A key area of this is the escalation of delays or
potential delays both within the Trust and externally to senior leads within the
local ICBs and Social Care to always ensure a transparent sitrep.
The post holder will provide specialist discharge advice to multi professional teams, patients, and their advocates. They will also be responsible for identifying gaps in knowledge regarding discharge and providing training and development to hospital staff. Included in this would be ensuring their knowledge is current and the dissemination of current information to all who require it.
The post holder will be responsible for a case load of the most complex patients and will be responsible for completing and coordinating assessments and discharge planning for this group of patients. This will include discussions and escalations to senior managers across a broad range of partnerships, challenging conversations and involvement with the trust legal team and solicitors.
This group of patients Includes but is not exclusive to
Homeless patients, ensuring that the correct pathway is followed and that the
trust are in receipt of any changes to legislation or the process
Complex Neurology patients requiring either specialist rehabilitation or
placement ensuring that the correct pathway and funding streams are applied
for and adhered to incorporating any changes to legislation or process
Overseas patients with no access to public funds requiring intervention from
the Home Office or repatriation to their country of origin
The Senior Complex Discharge Navigator will manage the band 6 Complex Discharge Navigators will assist with managing the Patient Flow Coordinators ensuring their training and appraisals are up to date.
