An exciting opportunity has arisen within the Community response and rehabilitation team Team (CRRT), for an enthusiastic, dynamic and motivated Advanced Occupational Therapist to work alongside the Community based MDT team lead to provide clinical expertise and leadership. You will be working alongside and Multidisciplinary Team of experienced, Physiotherapists, Occupational Therapists, Nurse Specialists, Associate Practitioners, Support Workers and administrative staff.
• This post requires the applicant to have significant experience at Band 6 or above working with Multidisciplinary teams within community services.
• You will be expected to work autonomously within community settings assessing, managing, and treating patients, with the aim of preventing admission to hospital, facilitating timely and safe discharges and providing rehabilitation to patients within the community.
• You will be expected to work between community sites whilst predominantly visiting patients within their own homes.
• The service works over 7 days, with weekend and bank holiday commitments.
• Flexible working pattern to cover a 7 day service from 8:00am to 8:00pm

The key objectives in this role are:
• The role would suit an individual who enjoys variety through their week and contributing to a team approach to discharge planning, admission avoidance and rehabilitation
• The post holder will have an attention for detail and bring skills of rehabilitation and assessment into their caseload management
• To assess and engage patients in the community, to attain optimum levels of safety, independence, health and wellbeing
• Support team lead, use of admin systems including Allocate, sel expenses and datix will be required

• To provide a high standard of occupational therapy care to non-complex, complex and highly complex patients within specialty evidencing high-level problem solving, reasoning skills and independence of judgement.
• To lead the development and delivery of enhanced rehabilitation, reablement, equipment and adaptations pathways working across a range of community services and in-reaching into the hospital.
• The as Advanced OT will be a highly specialist practitioner and is the clinical lead for the Occupational Therapy Service within specialty providing highly specialist knowledge within the Trust and externally to therapists and other health care professionals from secondary and primary care across the region.
