Wiltshire College & University Centre have a great opportunity for an Education, Health & Care Plan (EHCP) and Transition Coordinator to join our team.
Location: Lackham, Wiltshire
Salary: £27,288 per annum (Professional Services Grade 6)
Job Type: Permanent, Full Time (37 hours per week/52 weeks per year)

EHCP and Transition Coordinator - The Role:

EHCP and Transition Coordinator - Key Responsibilities:
- Transition Coordination: Collaborate with Specialist Coaches and Campus Inclusion and Support Managers to liaise with admissions and local schools, ensuring seamless student transitions.
- Team Support: Organise the transition team to guarantee a consistent, high-quality experience for students across all campuses.
- Student Monitoring: Track student attendance and progress, promptly flagging any concerns to Inclusion and Support Managers and curriculum colleagues to implement timely support and interventions.
- Annual Reviews: Lead on high-quality, person-centred reviews. This includes scheduling, inviting attendees, collating and distributing reports, and working with teaching staff to ensure comprehensive review documentation in line with statutory guidance.
EHCP and Transition Coordinator - You:
To apply, you will hold:
- Level 2 (GCSE A-C) qualification or equivalent in English and Maths.
- A level 3 qualification in a relevant subject or equivalent knowledge and experience.
- Strong interpersonal skills with a commitment to a person-centred approach.
- Thorough understanding of SEND and the statutory requirements for supporting young people and their families.
- A keen interest in developing and enhancing departmental systems to better meet student needs.
