Develop a caseload to support the needs, abilities and capacities of children, young people and their caregivers/families.Collaborate with the referral source and the patient/caregiver/family to develop and implement a patient-centered service plan to meet their social, emotional and developmental needs.Document service provided and provide written reports in accordance with established policies and procedures.Organize and undertake home visits to patient/families and carers following referrals from the diabetes team.Document patients, caregiver/familys capacity and support them with managing diabetes.Communicate any findings/concerns or patients, caregivers/familys questions to the team.Facilitate access and referral to appropriate services for additional support that may be requiredSignpost to appropriate parenting education courses and materials for parenting, managing conflict, and motivation in partnership with other professionals in the Diabetes teamOperate in accordance with the Trusts child protection policies and procedures. Makes appropriate referrals and supports the Trust social work department in statutory child protection procedures where required.Maintain an awareness of community resources, including health care services, social, economic, recreational, employment and educational services and resources. Establish and maintain coordinated working relationships with community social services departments, other professionals and service providers involved with children and families. Assist patients and caregivers/families to access and utilise services.Participates in staff MDT meetings, virtual clinics.Support and lead CAF process where required, attend case conferences. Participate in quality improvement initiatives and committees, and maintain up-to-date knowledge through involvement in appropriate work-related continuing education as required by the employer.Meet the patients/ families and carers in the hospital, at the community or conduct telephone interviews as appropriate in order to provide a productive and flexible service.Maintains accurate records of all contacts, using the Trust electronic database systems.Demonstrate excellent communication skills, verbal and writtenAt times will work without direct supervision and work within set parameters and understand operating procedures.Be fully aware of the potential hazards or risks which may face a lone worker and act in a way that ensures personal safety and the safety of others.Participate in regular clinical supervision with supervisor and initiate the same when requiredWork in collaboration with service users and carers and to acknowledge and value the contribution they make. Undertake training to develop a range of knowledge and skills in order to deliver high quality clinical interventionsRecognize and appropriately address risk factors to children and young people and carers within their healthcare setting and feedback appropriately to the registered practitionerSupport good health for all children and young people within the local community.Prioritise own tasks under the appropriate delegation of the registered practitioner.The post holder will be required to take part in an annual performance appraisal, where this job description will be reviewed, and objectives set.Take appropriate action in emergencies and unexpected events, i.e. contacting on call emergency services, team members etcThe post holder must be able to independently travel around the community
