2. KEY REQUIREMENTSProfessional2.1 Work in partnership with locality GP practices to support the uptake and delivery of annual health checks and participate in the annual health check audit process.2.2 Work collaboratively with multi professional agencies, to ensure that access to health care meets the individuals health requirements across the adult life span, including long term co-morbidities, deteriorating health and end of life care.2.3 Participate in the processes of incident reporting, incident reviews and learning from deaths (Learning Disabilities Mortality Review).2.4 Address health inequalities and ensure the implementation of the different legislative and human rights-based frameworks underpin nursing practice.2.5 Contribute to national and local initiatives and research to steer organisational best practice and service improvement.2.6 Advocate for and contribute to public health initiatives to promote understanding of inequalities and the needs of people with a learning disability.2.7 Develop clear and effective partnership approaches to ensure the full range of healthcare needs for people with a learning disability are met by the most appropriate service and agencies, including voluntary and charitable sectors. 2.8 Promote and facilitate mainstream health education and health promotion activities including access to annual health checks and appropriate health screening.2.9 Provide specialist education and training to a range of professionals (alongside the PAMHS lead).2.10 To work closely with screening programme leads to develop appropriate reasonably adjusted care pathways.2.11 Provide support to other professionals to ensure a consistent and seamless health care approach across health and other care sectors for all individuals.2.12 Through comprehensive assessment, develop specialist care plans to ensure complex, physical health, social, psychological, behaviour with challenges and medical interventions are fully integrated.2.13 Monitor outcomes of care planning using evidence-based frameworks that can positively influence timely and prioritised approaches to improve health outcomes for individuals2.14 Liaise with other services to ensure reasonable adjustments are in place for health appointments and consultations, including preparation for unpredictable behaviours2.15 Ensure that capacity and best interest decisions involve the person with the learning disability in a way that they can express their views and understand the decision to be made.2.16 To adhere to the NMC Standards of Conduct, Performance and Ethics for Nurses and Midwives.2.17 To teach and mentor nursing students.2.18 To participate in formal supervision as a supervisee and provide formal supervision to the team health care support workers.2.19 To maintain contemporaneous records using both manual and electronic systems.2.20 To actively participate in personal, team and service development.2.21 To work under the guidance of the PAMHS lead nurse. 3.0 EDUCATIONAL/RESEARCH3.1 To participate in the delivery of relevant training, education and advice to clients; health professionals, carers and families.3.2 To assist in clinical audit and client satisfaction surveys.3.3 To participate in multi-disciplinary working with both internal and externalcolleagues.3.4 To keep abreast of learning disability and primary health care strategies(national and local) to ensure that high quality evidence based approachesare utilised.3.5 To contribute to the support of student nurses, providing training, coaching and mentorship during placements. 3.6 To undertake mandatory and specialist training in keeping with Trust policy. 3.7 To attend meetings and conferences when required, to update oneself in new developments in epilepsy and health promotion. 4.0 MANAGERIAL4.1 To ensure appropriate governance and risk management arrangements are inplace for caseload and to maintain the safety of clients, staff and others inaccordance with relevant regulations, policies and procedures.4.2 To assist the team manager in the delivery of the service, ensuring safe,effective and efficient interventions are maintained.4.3 To supervise and direct the work of the health care support workers.4.4 To ensure the most effective and efficient use of resources and equipment.4.5 To provide statistical information and other relevant information as required. 5.0 GENERAL5.1 To participate in meetings and development groups relevant to the service orthe Division as required.5.2 Develop and maintain good communication with managers; professional andnon-professional colleagues and external partners.5.3 Provide advice and support to the Inpatient Service as required or as directedby the Service Manager.5.4 Work to the values and visions of the Trust and the Division, keeping clients at the centre of positive, meaningful and compassionate interactions. This will be in keeping with current best practice principles and will include positivebehaviour support approaches.5.5 Maintain a polite and positive manner with colleagues; families; managers;independent providers; advocacy services and contractors.5.6 Report any accidents or incidents using approved systems and processes;escalating serious or untoward incidents to a senior manager in accordancewith policies and procedures.5.7 Maintain an awareness of local and Trust policies and procedures.5.8 To undertake any other duties of a similar nature consistent with theresponsibilities of this post in order to provide a quality PAMHS service.
