Crynodeb
This is a 10 PA post for a Specialist Grade Geriatrician based in the Urgent Community Response and Hospital at Home service at the Royal Surrey NHS Foundation Trust. This is a full-time post in Geriatric medicine working across the acute and community interface. The Royal Surrey NHS Foundation Trust is looking to fully integrate acute and community services to create one seamless pathway for older adults living with frailty and complexity. While any combination of job plan is possible, we anticipate the post holder to provide senior oversight for the integrated Urgent Community Response and Hospital at Home service. The Hospital at Home team is evolving and developing rapidly. The current caseload is managed by a Consultant Geriatrician, Advanced Clinical Practitioners (both qualified and trainees), one or two middle grade doctors and the broader multidisciplinary team including physiotherapists, nurses, pharmacists and clinical rehabilitation assistants.
