A Vacancy at Guy's and St Thomas' NHS Foundation Trust.
An exciting opportunity has arisen to work within our team at the Amputee Rehabilitation Unit (ARU) as discharge facilitator. This role is for someone with a genuine and proven enthusiasm in discharge coordination for complex patients.
The ARU is part is a specialist inpatient rehabilitation facility that provides intensive amputee rehabilitation to those who have been affected by a major amputation of one or more limbs.
You would be joining a close knit team lead by our Clinical Lead AHP, and including our prosthetist, physiotherapists, occupational therapists, rehabilitation assistants, counsellors, nursing team and nursing assistants, medics and pharmacists. The ARU team is supported by the Consultant AHP based at the limb fitting centre (Bowley Close).
The successful candidate will be required to be an excellent communicator and able to work autonomously and hold their own caseload. You will need to be a good organiser in order to work in partnership to provide a seamless service and meet the needs of the team.

The post holder will be responsible for facilitating the improvement of the quality, effectiveness and efficiency of patient discharge planning processes on the Amputee Rehabilitation Unit (ARU).
The post holder will be required to act as a flexible administrative resource supporting the patient discharge planning processes from the unit. They will be required to lead on discharge planning process improvement projects thus enabling patients to achieve a safe, effective and timely discharge from the unit.
The post holder will be required to undertake non-clinical administration and organisational tasks and also patient facing tasks like assessments and communication in relation to discharge planning.
Other duties include coordinating and attending family meetings or case conferences, liaising with social services on packages of care, arranging equipment delivery and supporting with housing and benefits applications.
The post holder will be required to use of Microsoft Office for general office-based tasks, completion of audits, writing clinical notes and supporting with referrals amongst others.

The Amputee Rehabilitation Unit (ARU) is a 12 bedded inpatient rehabilitation unit, located at the Lambeth Community Care Centre, Kennington. The essence of the ARU model of care is to provide specialist rehabilitation for people who have undergone major amputation. Therapy is delivered over 6 days to maximise patient outcomes and goal attainment as an inpatient. Patients stay for a maximum of 49 days, when they will then be discharged home to continue rehabilitation as an outpatient.
The successful candidate will be an integral part of the MDT and have opportunities for internal and external training and development, in particular becoming expert in the management and discharge planning for complex amputees. We hold weekly in-service training, which is both uni and multi professional and we strongly encourage self-directed learning.
Work flexibly alongside both clerical and clinical staff to maximise the efficiency of patient discharge planning processes at the ARU.
To have a sound understanding of the field of patient discharge planning and patient experience
To lead discharge process improvement projects, identifying potential gaps in service and care
To collect, analyse and present data pertaining to efficiency of patient discharge processes
To be responsible for facilitating the patient discharge journey from admission to discharge, highlighting any delays in this process and ensuring adequate data is collected to support service development
