Summary
The post holder will gain experience in various sections of the pharmacy and provide a clinical Pharmacy services to Moorfields outpatients, inpatient and day-care services, and paediatric patients at the Richard Desmond Children's Eye Centre (RDCEC).
They will be responsible for ensuring that operational services are running smoothly from the pharmacy dispensaries and provide a specialist pharmacy service to Moorfields patients.
The post holder will also support in the development of trust wide medication incidents, medicines management, financial, and quality and safety reports.
The post holder will:
• Support the Senior Pharmacists in the delivery and development of clinical Pharmacy services throughout the Trust.
• Provide comprehensive medicines optimisation services to Moorfields Eye Hospital (MEH) patients.
• Provide specialist pharmaceutical advice on the safe, appropriate, and cost-effective use of medicines, support medicines optimisation initiatives, identify risk management issues and promote safe systems of practice.
• Improve the quality of pharmaceutical care to patients.
• Undertake regular audit to monitor and promote adherence to local and national guidance.
• Support and develop Trainee Pharmacists, Band 6 Pharmacists, and other Pharmacy staff as appropriate.
• Work with the Deputy Chief Pharmacist, Lead Pharmacists, Nursing and Clinical Leads and other relevant Pharmacy Leads on service developments and improvements to ensure that Pharmacy services meet the needs of the users.

THE POST
• Support the Senior Pharmacists in the delivery and development of clinical Pharmacy services throughout the Trust.
• Provide comprehensive medicines optimisation services to Moorfields Eye Hospital (MEH) patients.

MAIN RESPONSIBILTITES
Clinical Services:
1. To ensure that clinical Pharmacy support from the dispensaries ensure that:
• prescriptions are safe, effective and cost efficient,
• there is optimisation of medication for the individual patient,
• patients are counselled on their medicines including how to obtain further supplies,
• the service is provided in accordance with legislation (e.g. General Pharmaceutical
Council), national guidance and Moorfields policies.
2. To manage medicine queries as required.
3. To deliver support directly to the rotational clinical Pharmacists and Trainee Pharmacists in delivering a clinical Pharmacy service.
4. To provide clinical pharmacy support to MEH patients with complex medicines needs and to liaise with clinicians in managing these patients.
5. If necessary, to dispense and supply Investigational Medicinal Products (IMPs) in accordance with Clinical Trials protocols.
6. To dispense and supply High Cost Medicines in accordance with Trust policy.
7. To work with the Lead Pharmacy Technicians to manage the workflow through the Pharmacy dispensaries as required.
8. To be responsible for updating, maintaining and developing relevant policies, protocol and procedures that may be required to ensure the quality and safety of the services provided. These may be multidisciplinary.
9. To identify and deliver on audits related to the Pharmacy service and clinical audits to monitor and promote adherence to local and national guidance.
10. To ensure that medicine safety incidents identified are recorded as per Trust and department policy and to work with the Associate Chief Pharmacist – Clinical Services,
Governance & Medication Safety in analysing and identifying issues and remedial action.
11. To manage the storage and supply of Controlled Drugs including audit.
12. To provide operational cover as required by the service.
13. To work with specialist Pharmacist colleagues to ensure that clinical Pharmacy support is delivered to a consistently high standard.
14. Assist in the production of Patient Group Directions (PGDs), with appropriate Lead Pharmacists, medical and nursing staff.
15. To ensure staff observe safe working practices and comply with legislations i.e. Good Manufacturing Practice (GMP), COSHH Regulations, and Safe Handling of Cytotoxics.
16. To undertake any other duties commensurate with the post of Specialist Pharmacist where required by the Associate Chief Pharmacists or the Chief Pharmacist.
17. To ensure the service is managed in accordance with departmental policy and procedures. Service Development and Improvement:
18.To liaise directly with clinical staff to proactively manage medicines and develop systems to improve prescribing practice.
19.To monitor the clinical Pharmacy service and Key Performance Indicators (KPIs) to ensure that the service is consistently delivered to a high standard.
20.To provide medicines intelligence to identify and support measures to ensure that medicines are used in a high quality, safe and cost-effective manner.
21.To develop and produce medicine usage reports for selected clinical specialties and to work with the Lead Pharmacists to manage medicine spend within the different specialties.
22.To support the identification and implementation of Cost Improvement Plans (CIPs). Managerial, supervisory and training responsibilities:
23.To provide training and clinical assessments for Pharmacy staff working within the Pharmacy department.
24.To work with the Lead Pharmacists and Senior Pharmacy Technicians to develop the roles of staff working within the Pharmacy department.
25.To work with the Senior Pharmacy Technicians to manage staffing levels in the dispensaries to ensure that sufficient support is provided.
26.To be a contact point for Pharmacy related Education and Training matters both within and external to the department.
27.To contribute to the in-house training and development of clinical Pharmacists and Trainee Pharmacists and to ensure that the training needs of all team members are met.
28.To act as a Trainee Pharmacist tutor, if required.
29.To participate in multi-disciplinary team meetings and Pharmacy medicines awareness training for nursing staff, when required to do so.
30.To set annual objectives, personal development plans and to regularly review progress of junior staff within Pharmacy.
31.To be an Educational Facilitator for clinical Pharmacists undertaking the Postgraduate Diploma in General Pharmacy Practice. This involves evaluating practice and assessing the practitioner as outlined by the programme provider.
32.To develop and motivate staff to ensure they can and do perform well in their job, contribute towards improvements to the pharmaceutical service by:
• Encouraging staff to suggest and implement changes to improve pharmaceutical services.
• Encouraging an open attitude to performance, and ensure a no-blame culture to enable review of past performance and implement change.
• Being responsible for the authorisation of leave, ensuring adequate cover is provided in their absence and efficiency of the service is maintained.
• Ensuring adherence to Trust sickness policy, absence monitoring system and conduct return to work interviews.
Personal Development:
33.To undertake and participate in the in-house, regional education training programmes and other activities to develop skills as part of a commitment to develop a Continuing Professional Developments (CPD) portfolio to meet all requirements of the General Pharmaceutical Council (GPhC).
34.To maintain Trust mandatory training compliance.
35.To progress along the Advanced Pharmacy Framework.
36.To maintain a commitment to self-development and undertake further training opportunities as identified through appraisal and performance monitoring system.
