We are excited to announce an opening for a diligent and experienced Care Coordinator. We are looking for a passionate professional to enhance our dedicated team and contribute to our mission of delivering exceptional care services to our clients.

As a Care Coordinator, you will play a crucial role in ensuring that our clients receive the best possible support tailored to their individual needs. Key Responsibilities:
• Coordinate and oversee care services for clients
• Schedule and assign care staff to ensure efficient delivery of services
• Conduct client assessments to determine care needs
• Communicate effectively with healthcare professionals, clients, and their families
• Maintain accurate and detailed care records
• Monitor and evaluate care plans to ensure high-quality services
• Supervise and instruct staff members on all aspects of their work, giving help and guidance where appropriate and assisting manager(s) to keep records up to date.
• Undertake scheduled spot checks and supervised visits on carers work and feedback effectively to support the training and development of team members.
• Undertake client visits & reviews in line with the company’s policies & procedures to facilitate a professional & personal care service which meets the client’s needs.
• Deliver care services to clients to cover absence and sickness.
• Take part in the on-call rota to offer support to carers and clients during non-office hours.
What We Are Looking for In a Successful Candidate:
• Proven experience in care coordination or a related role
• Experience in care management and experience in rota scheduling
• Proficiency in word processing, email, and calendar software
• Organised with the ability to multitask
• Good communication with the ability to build and maintain strong relationships.
• Flexible and able to cope with changing demands, priorities and remaining calm under pressure.
• Friendly, confident and helpful telephone manner.
• A strong focus on teamwork, and the ability to work independently and apply initiative.
• A positive, focused attitude to enable targets to be met and exceeded.
• A full UK driving license and own car is essential.
Qualifications:
