2 x Diabetes Specialist Podiatrist and Team Leader- Band 7 Full-Time
An exciting opportunity has arisen for 2 Band 7 qualified Clinicians to deliver Specialist care for high risk/diabetes foot care in the community setting of the borough of Ealing.
This post requires a person who is highly specialised and has extensive knowledge, clinical skills and experience at working at a level in the field of Diabetes. Providing comprehensive care for patients with complex high risk foot problems. Providing education to health care staff and patients on management of diabetes and diabetic foot care.
The post holder will be clinically responsible for treatment of the high risk caseload working autonomously as well as work as part of the wider Podiatry team. You will be expected to lead on diabetes and provide support and education to junior staff, other HCPs and your peers.
The post holder will also line manage and supervise junior staff members within the team; delivering supervision, sickness meetings and support to that staff member. You will also deputise on behalf of the service manager on a rotational basis.
ToprovideDiabetesspecialistfootcareforpatientswithcomplexhighriskfootproblems.Thisincludes
diabetespatients and non-diabetic patientswithacuteandchronicfootwounds, patients with neuropathic and vascular complications, patient withfoot complications who are immunosuppressed and patients who have poortissue viability.The post holder will be clinically responsible for treatment ofthe high risk
caseload working autonomously as well as working/liaising with themultidisciplinaryteam and other health care professionals involved in the patient’s care.
The postrequires accepting emergency referralsfrom GPs, community/acute podiatrists, and other health professionals and willprovideresponsivespecialistcare. There will be a need for education and up-skilling of clinical and care staff.Therewillalsobearesponsibilityforpatienteducation and working with education programmes and competency frameworks within the trust i.e Type 1 and type 2 education, along with upskilling band 4, 5-6, 6-7, apprenticeships, students, work experience, cadets, careers days and volunteers.
As part of the Podiatry service, the post will be based in a varietyofsettingswithinEaling Community Partners. Car driver is desirable for this role as you will be required to carry out home-visits.

Diabetes Lead:
1. To assess,manageand treathigh risk complex caseload consistingof patientswith: acute and chronic diabetic foot problems, pre and post-operative amputationswoundsandacuteandchronicneuropathic,vascularfoot/lower limbproblems, Charcot and other diabetes related MSK issues.
2. To lead in the planning of care packages jointly between hospital and communitycare providers.
3. To provide podiatry care including wound care for patients with musculoskeletalconditions.
4. Touseclinicalreasoningskillintheassessment,diagnosesandformulatingtreatment plans of patients with a variety of diabetes high risk foot problems and other medicalconditions.
5. Tohavethe abilityto cope withunpredictable workpatternsas necessitated incommunity care clinics.
6. Develop and contribute to the care and management of complex wounds and lowerlimbcomplications.
7. Toapplyandpracticeprinciplesofwoundmanagement according to the national and local competencies.
8. Have advance knowledge of dressings and the modes of action and appropriateindicationsof theirusein wound care.
9. To have a good working knowledge of high risk biomechanics and its role in wound care andbasicmusculoskeletalproblems.
10.To recognise the need to request and understand the result of investigations suchasall biochemistry includingmicrobiology, x-ray,blood resultsandscansand to respond appropriately, by prescribing or requesting prescriptions.
11.To take the lead as part of the clinical diabetes/high risk foot specialist role within the community services and attend network, steering and wider meetings and groups.
12.To provide diabetes/high risk foot care within the community setting with links to the secondary care and other care services and set up meetings to work in collaboration.
13.To ensure effective communication of complex sensitive and emotional informationtopatientse.g.ulcercares,amputations,Charcot’sandvascularcomplications.
14.To be able to recognise hypo/ hyperglycaemia and manage it appropriately and tobe able to refer patients directly for diagnostic tests, such as, bloods and to interpret and act accordingly on data provided therein
15.To refer on urgent/emergency or exceptional cases to other healthcare professionals and communicate (in writing or emails) with other health or social care colleagues to ensure the patient receives other services/ care required.
16.Topromotemultidisciplinaryteamworking. To work in close collaboration with the specialist team including diabetologists, vascular surgeons, specialist diabetes nurses/dietitians and tissue-viability specialists in all care settings to ensure a holistic approach to the management of the at-risk and high-risk patient is met.
17.To support the podiatry clinical network in developing and delivering the CPD training and developing training programme in the specialist area of diabetic and ischaemic foot care.
Managerial/Leadership:
1. To actively participate in and at times lead team meetings as appropriate.
1. To play an active role in the promotion of quality & safety within the clinical environment.
1. Represent Podiatry service at local and regional meetings as required.
1. To undertake clinical and managerial supervision with staff members designated to you and deputise for the service manager on a rotational basis with other leads in the service where appropriate.
1. Totaketheleadininterpretingcorporatepolicyandco-creatinglocalguidelinesforand withtheteamto follow.
1. To lead the team of clinicians and support staff in the provision of pro-active high-quality podiatric care and supporting peer reviewing of staff.
1. Tobe activelyinvolved in therecruitment,selectionand localinductionofstaff.
1. Responsible for organising own workload and responsibilities to meet personal andorganisationalobjectives and using your experience, knowledge and skills to make clinical judgements. Havinghighlydevelopedtimemanagementskillstocreateappropriatebalancebetweenclinicalandmanagerialroles.
1. To use the non-clinical time appropriately by using it for but not restricted to audits/research/projects/lead work/meetings/education and training and recording this in the daily rotas.
1. Work with the Service Manager to set work for staff around financial targets, stockmanagementandclinicaloutcomes.
1. Toliaisewith ServiceManagerinsettingupnewservicedeliveryprojects.
1. To undertake any other duties that might be considered appropriate by the Service Manager.
1. To participate in the services Clinical Governance initiatives and the development and implementation of standards, guidelines and protocols.
