ANAESTHESIA AND INTENSIVE CARE Critical Care Services The Intensive Care Unit has 16 adult beds; these beds are run flexibly, admitting level 1-3 patients, as well as post-operative surgical patients for enhanced level care. Medical care is currently coordinated by a team of 10 consultant intensivists five days per week between 8am and 8pm and at weekends from 8am to 2pm. The working week is split to facilitate work/life balance and family life, as well as optimal continuity of care. The annual admission rate is around 800 cases, although the figures have varied somewhat because of the COVID-19 pandemic. As previously noted, a new, purpose built, critical care unit is now open with any facility you can think of to support staff well-being and patient care. There is a full remit of modern ultrasound machines, with several ICU consultants being fully trained FUSIC mentors running regional and national ultrasound courses which are the best in the region. We highly encourage arranging a visit to see what the facility has to offer! The Intensive Care Unit has recently become fully digital with its own bespoke integrated clinical information system ICCA (IntelliSpace Critical Care and Anaesthesia). This provides advanced clinical decision support software, standardized documentation and enhanced audit capability, all allowing for improved patient care. The Critical Care Team provides a dedicated, multidisciplinary consultant led outpatient ICU Rehabilitation follow up service and have helped guide critical care units around the region who also wish to set up this service. In addition, we have a Vascular access service for patients who require long-term venous access, enabling antimicrobial or parenteral feeding administration. There is a Trust wide Acute Response Team. This team of Advanced Nurse Practitioners provide a 24-hour outreach service and has provided inspiration and modelling to neighbouring Trusts on how to run a service that not only reduces critical care admissions, but also follows up discharged patients and provides education and support on the wards. The intensive care consultant team are a dynamic, forward-thinking team of clinicians, most of whom have additional managerial roles in the trust or nationally. Two are RCOA examiners and one is a FFICM examiner and is the chair of the Intensive Care Society Standards and Guidelines Committee. Two consultants are FICE/FUSIC mentors. We work closely together and are looking for a similarly keen, dedicated, and skilled clinician to join our team. Anaesthetic and Operating Services The Trust is an acute admitting trust with a modern and busy Emergency Department and this, along with the emergency theatre work is supported by the critical care and anaesthetic team. The Anaesthetic Department provides a full range of anaesthetic services for general surgery, urology, orthopaedics and trauma surgery, gynaecology, ear nose and throat, ophthalmology and oral surgery. There are 8 (soon to be 9) in-patient operating theatres and an obstetric theatre attached to the labour ward. Main theatres are situated in close proximity to the ICU, which is also near to the theatre recovery area. The Trust has a nurse led pre-operative assessment unit and we provide weekly CPET testing for all patients undergoing major colorectal surgery. All the main specialties utilise a dedicated Day Surgery Unit with a further two operating theatres, which provides an excellent and efficient environment for undertaking some of the day case workload. Two new minor ops rooms have recently opened which will allow us to expand our delivery of surgical services. The Anaesthetic Department provides a full range of services to the Obstetric Unit where there are approximately 3000 deliveries per year. There is a 24- hour epidural service. During the week, daytime anaesthetic sessions on the delivery suite are covered by consultant anaesthetists with a special interest in Obstetric Anaesthesia. Elective Caesarean sections are performed on two lists per week, the vast majority under regional anaesthesia. There is a dedicated operating theatre on the delivery suite for all emergency Caesarean sections and other operative procedures.
