An exciting secondment opportunity has arisen to join an established team of Community Children’s Nurses in Birmingham. This is a city wide service that offers specialist nursing support to children & families with a variety of complex physical health needs.
We provide clinical interventions to children within their own homes, schools and other community locations. The service operates seven days a week and also provides an out of hours ‘on-call’ service.
If you are an experienced Children’s Nurse with excellent clinical skills, have advanced communication abilities you may be who we are looking for to join our busy team. This post will provide the successful candidate with the opportunity to develop rewarding therapeutic relationships with children & families in a community setting.

1. Work in accordance with NMC Code of Professional Practice and Scope of Professional Practice. Professionally and legally accountable and responsible for all activities.
2. Provide comprehensive evidenced based and skilled nursing care to children within their own home and/or other community settings.
3. Ensure assessment, planning and delivery of holistic care is of the highest standard utilising an agreed nursing model.
4. Work in partnership with children, families and carers to provide optimum individual outcome acknowledging children and family’s personal preferences and lifestyles.
5. Liaise and work in collaboration with acute services, primary healthcare team and other agencies when planning and implementing programmes of care.
6. Provide the clinical lead in care packages funded by the Community Complex Care team when allocated to the Community Children’s Nursing (CCN) caseload.
7. Provide health education information to the child/young person, family and carers to promote a healthy lifestyle through health promotion activities as required.
8. Plan and participate in teaching packages to families and carers to ensure evidenced based care is delivered.
9. Advise the Team Leader, Clinical Lead or Service Manager of any circumstances which give cause for concern in relation to patient care and/or any concerns raised by the primary healthcare team or partner agencies.
10. Undertake home visits to children registered with GP’s within Birmingham as required.
11. Undertake home visit risk assessments to ensure safety during visits especially those during unsociable hours.
12. Obtain informed consent from Children, Parents and Carers prior to undertaking care.
13. Maintain clear, concise and contemporaneous records adhering to BCHC record keeping policies.
14. To be professionally and legally accountable for nursing practice and all activities.
15. Provide a flexible service to accommodate the working environment.
