Band 7 Discharge Team Nurse Manager within the IDT Team @ WTWA
Discharge Team as Complex discharge coordinator Lead cross site at Wythenshawe and Trafford General Hospital.
You will provide clinical leadership and advice to staff pertaining to all matters of discharge, and safe patient flow.
You will maintain a managerial and operational overview of the hospital discharge processes in support of the Patient Flow Lead and IDT Operational Manager.
You will be working closely with Social Care and The Local Care Organisation.
The successful candidate will need to demonstrate excellent time management skills, leadership and problem-solving skills, someone who looks at the bigger picture and has an open, adaptable “can do” attitude. You will need to be confident using your own initiative whilst working under pressure in an ever changing, fast-moving environment.
You will need to bring, effective team working skills, and have an ability to influence and engage with staff from a range of professional backgrounds to support timely and safe discharge planning.
A knowledge and understanding of Discharge to Assess (D2A), NHS Continuing Healthcare, Complex Discharge Planning, and the Care Act 2014, Specifically Annex G.
You will also have the Band 7 managerial responsibility of the team for all aspects of HR, training and development. Ensuring the team complies with trust values and behaviour’s as well as maintaining trust KPI’s.
Role Summary
The post holder will be expected to provide specialist knowledge and care to the patient and their careers. They will support colleagues in the effective management of patients in their specialist area who require a high level of judgement, discretion and decision making within an inpatient setting to ensure. The post-holder will provide education and training to patients, corers, and healthcare professionals.
