Job Purpose

As Integration and Partnership Locality Lead, you will work with our operational social care teams, the NHS and other community partners to design and deliver the integration of health and social care. This will include:

  • Helping to develop, align and deliver the priorities at a countywide and system level, translating the Adult Social Care Business Plan and ICS strategies into practical action to change the lives of residents.
  • Identifying and sharing learning and best practice from across Essex and the rest of the UK on integrated models of care, partnership working and the Better Care Fund.
  • Developing the tools, frameworks and common approaches that support development, delivery and evaluation of Better Care Fund projects.
  • Connecting and collaborating with functions across the organisation to maximise impact from our system and partnership working.

The role is essential to supporting the development of relationships with key partners across the local system and identifying best practice in integration. A proven track record of influencing and the ability to build strong collaborative working relationships across the service to support operational delivery are essential in this role.

Responsibilities will include…

  • Work on a locality basis, supporting the Director for Integration and Partnerships, Head of Integration and Partnerships and ASC Directors, to design, develop and deliver new models for integrated and collaborative working with NHS and other partners
  • Leading on project delivery for locality integration projects, working with local partners
  • Encouraging innovation and horizon-scan of models of partnership working nationally and internationally to continually improve the service offer within Essex.
  • Advising and supporting on developing local partnerships in Essex with key partners including, Alliances, ICS, District and Borough Councils, NHS Community and Acute Trusts, voluntary and independent sector organisations, developing integrated delivery models to support the delivery of commissioned outcomes.
  • Ensuring consistency across the County, sharing best practice on partnership work and integration models.
  • Connecting with other functions across ECC to support, develop and co-ordinate partnership initiatives to tackle the determinants of health and address health inequalities and input into Integrated Care System meetings and locality meetings such as Alliance Boards and Local Health and Wellbeing Boards
  • Producing regular reports on all aspects of service delivery, quality and performance, including progress against Better Care Fund and improved Better Care Fund priorities.

You will bring…

  • Educated to degree level or equivalent.
  • Experience of working with stakeholders and partners to agree and deliver appropriate commercial structures that engender delivery.
  • Strong influencing skills with an ability to proactively lead and deliver change within a complex multi-agency environment.
  • An understanding of the broader integration of health and social care and implications for delivery planning at local level.
  • Strong leadership and behavioural skills with the ability to focus on results, forging a highly performing and motivated team from diverse backgrounds to achieve strategic objectives.
  • Proven ability of developing solutions to deliver quality service to customers with financial constraints and competing priorities.
  • Proven track record of using professional expertise to deliver objectives and outcomes while managing conflicting priorities.
